docs(batch2): fix plan writeback gaps

Write back the two drifts the ledger required: new-file commits need an explicit stage first, and the 251/276 caliber is mixed.

Also close review minors M-1 (self-contained repro commands) and M-4 (seven layer classes, not five). Annotations only; original sentences kept.
