I also advise to add a YAML preamble with the copyright, license and authorship in each file. YAML is not rendered in any interpreter and adds some useful machine-readable fields and meta information. Better if it's in SPDX format

It would be nice to have some example(s). Also for some doc formats (e.g. microsoft) I'm not sure this would work....so it may not be possible to enact universally. It would also be quite a bit of work if enacted on all current content non-legacy content (and I'm not sure who would do it), so a OC wide policy would need to be discussed.
