Rechecked against the widget code on latest main f8b4c464. The regular home-screen widget has no registered layout or timeline; the app currently registers only the separate Live Activity layout.

On our isolated iOS 26.5 simulator, adding the medium widget, opening the app and returning Home leaves the widget broken. The Debug extension displays a red placeholder, rather than your black release-build screenshot, so this is not a pixel-identical reproduction of your iOS 26.3.1(a) setup.

Jake Leventhal's existing #6464 fixes that initialization path. I verified the same installed WidgetKit widget renders "No active agents" with the candidate, including the small-widget control. Matched native before/after images are in that PR. I also fixed a preference-loading/sign-out race found during its review; all 61 focused tests pass locally.

Keeping this report open while the PR's new-head checks and human review of its widget design and refresh behavior finish. No separate duplicate PR or closure is needed.
