refactor(ui): 登记行键改为文件与档位二元组
`SURFACE_TAG_REGISTRY` 的键从 `file` 改为 `(file, tier)`（T19 第一步：键形态）。 条目类型用**显式必填**的 `tier: "legacy" | "new"` 取代 `legacy?: true`，29 行逐行补档位 （9 行 legacy / 20 行 new，`file`/`count`/`reason` 三列一字不改）；判据件的去重键（`REG_DUP`）、 字典序键与逐文件求和一律跟着键走。 不变量一字不动：登记行 29 · Σ 登记值 47 · legacy 9 行值之和 14 · 域内 `<Surface>` 实测 47 · 命中文件 29 · 锚（entries 29 / TaskConversationView.tsx = 3）同步。
