fix(progress-report): polish counts, stage 0 pending, calm no-log line - #454

- "done <date>" in a build-plan.md State cell counts as built in the plan
parser, so the Stage 5 card, the plan header and the phase badge agree
("not built" is still checked first).
- A placeholder gate-check failure on a stage after the current one reads
PENDING instead of FAIL.
- The no-session-log notice is one calm line; the regenerate hint moves
into the collapsed "How this was measured" block, and --no-log never
claims it looked in folders.

Small polish on the progress report, from a review of a fresh and an updated neutral demo project.
done <date>now counts as built in the plan parser (build-plan-status.sh), so the Stage 5 card, the plan header and the phase badge agree. "not built" is still checked first. The updated demo went from "3 built, 1 cannot read, Phase 2 unknown" to "4 built, Phase 2 in progress", matching the card's "4 of 6".--no-lognever claims it looked anywhere.
