Add /github my-issues to list open issues assigned to the requesting user. Keep results limited and sorted by last update.

Acceptance Criteria

  • Lists open issues assigned to a user (via configured mapping or explicit username option)
  • Defaults to a safe limit (5–10)
  • Sorted by updated time
  • Includes number/title/link in output
