chore: sync with upstream pingdotgg/t3code main - #144
Upstream's new label-gated relay preview workflows are taken as-is, matching how the fork already carries upstream's other deploy workflows; no fork divergence touches them.
Labeled same-repo PRs deploy stage pr-<n> behind the relay-preview environment reviewer gate. Unlabel or close tears the stage down, and a nightly job retries failed teardowns. Runtime-created managed endpoint tunnels and CNAMEs are swept by a PreviewTunnelSweep resource.
Known gaps, tracked as follow-ups: an interrupted preview deploy can leave runtime tunnels behind; repo-level relay secrets still bypass the environment gate until moved.
🤖 Generated with [Claude Code](https://claude.com/claude-code)
Medium Risk
Touches CI teardown with cloud secrets, pairing/protocol gates, and orchestration interrupt semantics; changes are well-tested but span infra and auth-adjacent paths.
Overview
Syncs a broad slice of upstream pingdotgg/t3codemain, led by relay PR preview automation and several client/server reliability fixes.
Relay previews: Adds relay-preview.yml (deploy pr-<n> when a same-repo PR is labeled relay:preview, gated on the relay-preview environment) and relay-preview-cleanup.yml (tear down on label removal/PR close, plus nightly retries). Preview destroys run a new Cloudflare tunnel/DNS sweep (previewStages, previewTunnelSweep) wired into alchemy.run.ts for pr-* stages only.
Stop / stale cache (pingdotgg#18188): After Stop, the client reconciles pending background turn items via getTurnItem, updates live state and expanded-thread IndexedDB snapshots, and does not block Stop on that lookup. Projection updates add single-index replacement hints so streaming structure comparisons stay cheap.
Pairing & SSH:t3 pairrefuses servers on a mismatched orchestration protocol before opening/migrating the local DB. Desktop SSH bootstrap maps [ssh_http:500] + version skew to a blocked “restart server on matching build” message instead of endless retries.
Orchestration: Run interrupts propagate reason from run_interrupt_request into results; MCP/agent stops use “Stopped by an agent” vs USER_STOP_REASON for user stops. Forked threads treat inherited fork history as “has conversation” for first /compact (with new tests).
Mobile & markdown: HTML <a> tags in tables become proper markdown links (safe hrefs only); Duo devices pin unstable_headerMinimizationBehavior: "never" on list screens to fix header stutter.
Preview recordings: Web reports browserRecordingFrameRate; server encodes agent recordings at the last reported rate (30/60).
Misc: Usage-limit banner treats expired snooze as inactive (“Wake now”); contracts move recording frame rate types to preview.ts; docs note preview workflow; fixture script seeds stale command cache for manual QA.

The relay preview workflows can expose production-capable credentials to anyone with write access who edits the workflow, and a concurrency setup that can leak preview stages. The SSH pairing change can also show a wrong "restart the server" message for unrelated server errors. These should be addressed or explicitly accepted before merge. The other findings are small client-state, documentation and message fixes.

Copy the request reason into results created before provider start.
When an agent stops a preparing or starting run with no provider turn, Line 9802 records the agent-attributed reason, but this branch writes "Run interrupted before provider start" to the result. The result therefore omits the stop reason. Use interruptRequestItem.message for the result message.

Workflow-level concurrency can drop a queued teardown.
Every labeled, synchronize and reopened event creates a run that claims the relay-preview-<n> group, even when the job if is false. relay-preview-cleanup.yml uses a job-level group for this reason.
Assume a deploy run is waiting for approval or still running, and a user removes the label. The cleanup job then queues in the same group. If the user pushes a new commit, a new deploy run queues in that group. GitHub replaces the pending cleanup job with it, so the teardown is canceled. The label is already gone, so the nightly retry never lists the PR. The pr-<n> stage leaks until someone removes it by hand.
Move the group to the job, so a skipped job does not take the pending slot.

The relay-preview environment gate does not protect repository-level secrets.
This workflow runs on pull_request for same-repository PRs, so GitHub uses the workflow file from the PR head. A user with write access can push a branch that changes this file and removes environment:. They can also drop the label check and add any step they want. The job then starts without approval and still receives repository-level secrets.
docs/operations/release.md lists CLOUDFLARE_API_TOKEN, PLANETSCALE_API_TOKEN_ID, PLANETSCALE_API_TOKEN and AXIOM_TOKEN as repository secrets. These secrets are therefore readable without approval. The reviewer gate only protects secrets that are defined in the environment.
Move the secrets this workflow uses into the relay-preview and relay-preview-cleanup environments only. Use preview-scoped credentials that are separate from the prod credentials. Remove the repository-level copies, and update deploy-relay.yml to read the prod credentials from the production environment. The PR description already lists this as a known gap. Close it before the label-gated flow can be treated as approval-protected.
Based on learnings: "flag reliance on Environment required reviewers as their sole safeguard: the actor can omit environment: and still access repository secrets".

Do not classify every version-mismatched HTTP 500 as a pairing incompatibility.
The /oauth/token handler maps internal access-token issuance errors to HTTP 500. The [ssh_http:500] marker is therefore not specific to pairing incompatibility. With a mismatched server version, this branch can convert an unrelated issuance failure into ConnectionBlockedError and tell the user to restart the server.
Classify the specific incompatibility response instead of relying on the HTTP status alone.

Document the configuration the preview workflows need.
Both preview workflows run in the relay-preview and relay-preview-cleanup environments. They read RELAY_API_ZONE_NAME, RELAY_TUNNEL_ZONE_NAME, CLERK_PUBLISHABLE_KEY, CLERK_JWT_AUDIENCE and CLERK_SECRET_KEY. This document defines those values only in the production environment, not at repository level. A maintainer who follows this document gets empty values in the preview runs.
Add the following to this paragraph:
The two environment names.
The reviewer requirement on relay-preview.
The variables and secrets each environment must define.
The paragraph also does not say that relay:preview must exist as a repository label.

Do not let setThread rewrite the connection status during reconciliation.
setThread recomputes status from awaitingCompletion and previous.error. With no error and waiting === false, it always writes "live". Reconciliation takes applyLock only after the getTurnItem lookups finish. setDisconnected does not take applyLock. This sequence causes the problem:
The lookups return settled items.
The connection drops, and setDisconnected sets status to "cached" and awaitingCompletion to false.
Reconciliation calls setThread(thread), which sets status to "live".
The thread then shows as live while it is disconnected. It stays that way until the next reconnect sets "synchronizing". A thread restored from the resume cache with status "cached" has the same problem.
Reconciliation changes only item statuses. Update data and keep the current status. Queue the persist with the same shouldPersistThread check that setThread uses.

Guard the indexedDB.open call against a missing database.
If t3code:connection-runtime does not exist, indexedDB.open(..., 4) creates an empty version-4 database with no thread store. This can happen if the isolated client has not cached any thread yet. The success handler then throws NotFoundError from db.transaction("thread", ...). That exception is not caught, so the returned promise never settles. The empty version-4 database also stays in the profile. The app opens the same version later, so it gets no upgradeneeded event and never creates its stores.
Abort the upgrade transaction when upgradeneeded fires. Reject with a clear message. Close the connection if the store lookup fails.
