fix(idempotency): reconcile the local governed crash window between the debit and its link - #306
Conversation
…he debit and its link The engine-independent finding from #302's 'Not addressed here' list. A local governed call charges with operation_key=<record_id> and only afterwards attaches that ledger entry to its idempotency record. A crash in that gap left response_json IS NULL and ledger_entry_id IS NULL, which matched neither reconciler pass: - the unstarted pass is scoped to operation_kind == 'upstream_mcp' - the stuck pass requires ledger_entry_id IS NOT NULL So the record stuck forever, replays kept raising in-progress, and needs_review stayed 0 — real money had moved and nothing surfaced it. Adds a pass that runs before the stuck pass and adopts the orphan by the key the charge already carries: if a ledger entry exists with operation_key == record_id, link it, and the existing stuck logic then repairs the record from its receipt or counts it for manual review. No new repair semantics. Deliberately NOT deleting the no-debit case. Local tools run their side effect before the debit exists, so expiring the identity would let a retry execute it a second time — the invariant asserted by test_stale_local_identity_is_not_deleted_without_compensation_proof, which caught exactly that mistake in an earlier revision of this change. Without a committed debit as compensation proof the record is left untouched. The session is created with autoflush=False, so the newly written links are flushed explicitly before the stuck pass selects them. Tests (both confirmed to fail against the unfixed source): - debit + receipt orphaned -> (repaired, needs_review) == (1, 0) - debit, receipt missing -> (repaired, needs_review) == (0, 1) Unfixed, both report (0, 0) — the record is invisible to reconciliation. Tests: 1709 passed, 32 skipped; production_trust 6 passed; ruff clean; mypy clean across 172 source files; demo and red-team trust-plane gates --assert clean. The linking pass adopted any ledger entry carrying the record's operation_key. The wallet/operation-key uniqueness constraint already makes a same-key credit impossible alongside the debit, so this was not reachable today -- but the pass's whole premise is "money left this wallet", and that direction should be stated in the query rather than inferred from a constraint elsewhere. Adds a negative test that plants a same-key credit and asserts reconciliation reports (0, 0) and leaves ledger_entry_id and response_json untouched. Confirmed to fail against the unguarded query (reports (1, 0): the credit is adopted as proof of a charge). …tention into a denial Second round of CodeRabbit review on #305. Both findings verified against the code; one is a bug I introduced in the previous commit. permit_write_contended was being stored as a terminal 403. reserve_budget raises it after retry exhaustion on a transient write conflict, and the handler I added one commit ago caught every PermitError and *completed* the idempotency record with a denial. That turns a momentary database conflict into a permanent stored answer: every retry of that key replays the 403 long after the contention cleared. It is now split -- transient contention abandons the uncharged record (so the key stays usable) and answers 503; only terminal denials complete as 403. Wallet spendability now travels with the guarded debit. The status check read a row loaded at the top of the transaction and the write happened several statements later, so a freeze committing in between was invisible and the debit went through. A freeze is the control that exists to stop spending at that moment, not at the next request. A zero-row result is classified so a freeze is reported as wallet_frozen rather than as an empty balance. Confirmed against the unfixed source by reverting the service, not the test: the frozen wallet is debited, balance_after 98.5, a real ledger entry minted. Not taken, with reasons on the PR: the daily-limit predicate (daily_spent is maintained by the velocity monitor in its own transaction and is already reversed on rejection; folding it into the debit predicate would double-count the interaction between the two and deserves its own change), and the amount < 0 filter on ledger adoption (moot -- that pass is removed here, see below). Also dropped this branch's orphaned-local-debit pass in favour of #306, which implements the same fix better: it runs *before* the stuck pass with an explicit flush, so the record resolves in the same run instead of the next, and it lets the stuck pass classify repaired vs needs_review instead of counting every adoption as review. Its test matrix is also stronger. No reason to carry two implementations of one reconciler pass into a conflict. Validation: 1788 passed / 32 skipped (app + b2a_sdk); ruff and mypy 2.3.1 clean over 173 files; doc references resolve.
#306 lands its own orphaned-local-debit pass in `IdempotencyService.reconcile_stuck_records`, running before the stuck pass with an explicit flush and classifying repaired-vs-review. That is the better implementation of the third pass this branch had carried briefly, so the merge takes it wholesale and this branch keeps none of its own. `tests/test_mcp_trust.py` conflicted only on two trailing blank lines this branch had added; resolved to main's version.
Summary
The engine-independent finding from #302's "Not addressed here" list — the one #304's SQLite guard does not mitigate, because no row-lock behaviour is involved.
A local governed call charges with
operation_key=<record_id>(app/routers/mcp.py:1705-1711) and only afterwards attaches that ledger entry to its idempotency record. A crash in that gap leavesresponse_json IS NULLandledger_entry_id IS NULL, which matches neither existing pass:operation_kind == "upstream_mcp"…ledger_entry_id IS NOT NULLSo the record stuck forever, replays kept raising in-progress, and
needs_reviewstayed 0 — real money had moved and nothing surfaced it.The fix
A pass that runs before the stuck pass and adopts the orphan using the key the charge already carries: if a
LedgerEntryModelexists withoperation_key == record_id, link it. The existing stuck logic then resolves it exactly as it resolves any other crashed finalization — repairing from the receipt, or counting it for manual review. No new repair semantics. What it deliberately does not do
An earlier revision of this change also deleted the record when no debit existed, mirroring the unstarted-upstream branch. That was wrong, and the existing suite caught it.
test_stale_local_identity_is_not_deleted_without_compensation_proofasserts the opposite, and the reason is in the reconciler's own comment: local tools have different side-effect ordering — the tool runs its side effect before any debit exists — so expiring the identity would let a retry execute it a second time. Without a committed debit as compensation proof, the record is now left exactly as found. That test is why this PR links only. It is a good example of the invariant paying for itself.
One implementation note
The session factory sets
autoflush=False(app/db/database.py:238), so the links written by this pass are still pending when the stuck pass runs its SELECT. They are flushed explicitly. Without that flush the pass silently linked the row but nothing repaired it in the same run — observed, then fixed.Type of Change
Validation
pytest -q -m "not production_trust"— 1709 passed, 32 skippedproduction_trust— 6 passed(0, 0)— the orphan is invisible to reconciliation(repaired, needs_review) == (1, 0)(repaired, needs_review) == (0, 1)← the case that most needs an operator: money moved, no evidence row to rebuild fromruff check .clean;mypy app/clean across 172 source filesdemo_trust_plane.py --assertandred_team_trust_plane.py --assertcleanRisk
Reconciliation-only; no change to the request path. The pass can only add a link that the charge's own
operation_keyalready proves, and it strictly widens what reconciliation reports — records that previously counted zero on both sides now repair or surface for review. It cannot delete a record, cannot debit, and cannot refund.Relationship to #304

Note
Low Risk
Changes only the background reconciler; it adds ledger links when a debit already proves compensation and does not alter invoke charging or deletion semantics for unproven local rows.
Overview
Fixes a reconciliation gap for local governed MCP calls that crash after the wallet debit commits but before
ledger_entry_idis written on the idempotency row. Those rows had no response and no link, so they missed the upstream-only “unstarted” pass and the “stuck” pass that requires a linked ledger entry—they stayed in progress forever and never showed up inneeds_review.reconcile_stuck_recordsnow runs an extra pass before the existing stuck logic: for idle local records on the governed MCP endpoint, it looks up aLedgerEntryModelwithoperation_key == record_id(the key the charge already uses) and setsledger_entry_idwhen proof exists. The unchanged stuck pass then repairs from the receipt or increments manual review. Rows with no matching debit are not deleted, so retries cannot re-run local side effects that happen before charging. An explicit
session.flush()after linking is required because autoflush is off, so the stuck pass can see the updated rows in the same run.Tests simulate the crash window and assert
(1, 0)when a receipt exists and(0, 1)when money moved but the receipt is missing.Reviewed by Cursor Bugbot for commit 3971a98. Configure here.
