Port of archived agentmiddleware/agent-middleware-api#829 (agentmiddleware/agent-middleware-api#829)

Changes (adapted to the monorepo layout)

 • gateway/app/db/database.py: SQLite connections use a cursor that closes a failed statement on the aiosqlite worker thread (fixes the Python 3.11 event-loop stall / "database is locked"), with gateway/tests/test_sqlite_failed_cursor_release.py.
 • .github/dependabot.yml: grouped updates, pointed only at real manifest directories in this repo.
 • .github/scripts/github_repo_settings.py and .github/scripts/github_branch_cleanup.py (dry-run by default) plus gateway/docs/github-org-configuration.md runbook.
 • Repo-name/link updates in gateway docs, openapi.json, integration and SDK READMEs.

Not ported

19 of 43 hunks applied; 24 skipped because the target file does not exist here (FINAL_SUMMARY.md, framework_integrations/, deploy-railway.md, trust-release-gate doc, secret_scan_range.sh, issue template), the change is already present (gitleaks pre-commit hook, README clone URL), or it describes the archived repo's history rewrite and does not apply here (.gitleaks.toml rewrite markers, api-key-rotation incident sections, ci.yml secret-scan base).

No migrations.

Tests run

 • pytest tests/test_sqlite_failed_cursor_release.py on Python 3.11 and 3.12: 1 passed each
 • pytest tests/test_docs_reference_integrity.py: 9 passed
 • ruff check / format --check on changed Python files: clean
 • both scripts --help: exit 0 (no GitHub calls)

Note

Medium Risk
New scripts can mutate branch protection and delete remote refs when run with --apply; changes are ops-only but misuse could affect merge gates or remove branches.

Overview
Adds GitHub operations tooling and documentation ported from the archived API repo: dry-run-first gh scripts to audit/apply repository flags, classic branch protection (preserving existing required checks and review restrictions), secret scanning, and to list or delete fully merged remote branches with SHA re-checks before delete.

Dependabot schedules now run Monday 06:00 America/Los_Angeles for Actions, pip, and npm. Python/npm grouping splits version vs security updates (replacing minor/patch-only groups). Docker gateway updates are grouped under gateway-images.

New gateway/docs/github-org-configuration.md runbook (linked from the docs index) documents script usage, Dependabot layout for this monorepo, gitleaks vs GitHub secret scanning boundaries, and an org-level UI checklist. test_github_admin_scripts.py adds offline pytest coverage for the admin scripts.

Walkthrough

The changes update Dependabot schedules and update groups. They add scripts that check and optionally update repository settings, and that report or delete eligible branches. A new runbook documents the scripts, dependency and secret-scanning configuration, and organization settings to verify.

Priority: Low

Estimated code review effort: 4 (Complex) | ~45 minutes

Merge Risk: Moderate · up to 50d5f

Fix the settings update failure before merging. Branch cleanup can still delete a branch that moves during an apply run, and its master-branch guidance needs correction.

Security architecture risk: Low · up to 50d5f

Changes are audit-only by default and require explicit application with existing GitHub permissions. The main risks are applying a single-maintainer review policy to a repository with stronger requirements and deleting a branch that moves during cleanup. Neither path establishes an unprivileged compromise.

Retained concerns

 • Medium · security · inferred: The new settings workflow applies a single-maintainer policy to any selectable repository: it replaces managed review settings with zero required approvals and no required code-owner review. On a target with stronger classic protection, this can reduce review gates. Dry-run output, explicit admin-authorized application and the documented rationale mitigate the risk; no evidence establishes that the default repository currently has stronger effective requirements.
 • Medium · reliability · inferred: Cleanup checks the scanned SHA immediately before deletion, but the DELETE request has no expected-SHA condition. A concurrent push after the final read can therefore cause deletion of a ref containing newly unmerged work, undermining the cleanup safety invariant and complicating recovery. Protected/base/preserved branches, explicit apply and repeated SHA checks narrow this acknowledged race but do not eliminate it.
Security review details

Security Blast Radius

 • inferred — Each invocation targets one repository, but --repo permits another target accessible to the authenticated account. Settings changes require repository administration authority; cleanup uses existing write authority. The demonstrated scope is repository settings and refs, not gateway tenant data, service credentials or organization-wide configuration.

Security Findings and Attack Paths

 • inferred — A security-relevant path requires an authorized operator to apply the policy to a stronger classic rule. The resulting zero-approval, no-code-owner target can remove those review requirements, although preserved checks, last-push approval or independent rulesets may still constrain merges. This is a conditional control-reduction risk, not evidence of an exploited or currently exposed repository.

Trust Boundaries and Controls

 • observed — Mutation authority comes from the operator's existing gh authentication, not from new application credentials. Explicit --apply and dry-run review separate observation from mutation. Cleanup excludes the base, protected branches and preserved prefixes; the settings tool enables secret scanning and push protection where GitHub permits them and reports entitlement failures separately.

Resilience and Maintainability Implications

 • observed — Cleanup processes failures per branch and returns nonzero when operations fail. Already completed deletions remain completed, and no durable recovery plan records their SHAs. Repetition rescans remaining remote state rather than rolling back the previous run.

Hardening Proposals

 • proposed — Preserve stronger approval and code-owner requirements by default, or require a separate explicit acknowledgement before reducing them on a selected repository.
 • proposed — For destructive cleanup, use deletion bound to the expected ref identity where supported, or require a period without concurrent writers. Persist a reviewed name-and-SHA plan to improve recovery from partial execution.

# REST API endpoints for Git references

Use the REST API to interact with references in your Git database on GitHub

## About Git references

A Git reference (`git ref`) is a file that contains a Git commit SHA-1 hash. When referring to a Git commit, you can use the Git reference, which is an easy-to-remember name, rather than the hash. The Git reference can be rewritten to point to a new commit. A branch is a Git reference that stores the new Git commit hash. These endpoints allow you to read and write references to your Git database on GitHub.

## List matching references

Returns an array of references from your Git database that match the supplied name. The `:ref` in the URL must be formatted as `heads/<branch name>` for branches and `tags/<tag name>` for tags. If the `:ref` doesn't exist in the repository, but existing refs start with `:ref`, they will be returned as an array.

When you use this endpoint without providing a `:ref`, it will return an array of all the references from your Git database, including notes and stashes if they exist on the server.
## Get a reference

Returns a single reference from your Git database. The `:ref` in the URL must be formatted as `heads/<branch name>` for branches and `tags/<tag name>` for tags. If the `:ref` doesn't match an existing ref, a `404` is returned.

Note

You need to explicitly request a pull request to trigger a test merge commit, which checks the mergeability of pull requests. For more information, see "Checking mergeability of pull requests".

Sometime in 2019^{1}, GitHub quietly hid the old `/repos/:owner/:repo/git/refs[/...]` endpoint in favor of two new endpoints that have the same functionality but provide consistent responses:

`--force-with-lease=`<refname>`:`<expect>

Usually, `git` `push` refuses to update a remote ref that is not an ancestor of the local ref used to overwrite it.

This option overrides this restriction if the current value of the remote ref is the expected value. `git` `push` fails otherwise.

Imagine that you have to rebase what you have already published. You will have to bypass the "must fast-forward" rule in order to replace the history you originally published with the rebased history. If somebody else built on top of your original history while you are rebasing, the tip of the branch at the remote may advance with their commit, and blindly pushing with `--force` will lose their work.

This option allows you to say that you expect the history you are updating is what you rebased and want to replace. If the remote ref still points at the commit you specified, you can be sure that no other people did anything to the ref. It is like taking a "lease" on the ref without explicitly locking it, and the remote ref is updated only if the "lease" is still valid.

`--force-with-lease` alone, without specifying the details, will protect all remote refs that are going to be updated by requiring their current value to be the same as the remote-tracking branch we have for them.

`--force-with-lease=`<refname>, without specifying the expected value, will protect <refname> (alone), if it is going to be updated, by requiring its current value to be the same as the remote-tracking branch we have for it.

`--force-with-lease=`<refname>`:`<expect> will protect <refname> (alone), if it is going to be updated, by requiring its current value to be the same as the specified value <expect> (which is allowed to be different from the remote-tracking branch we have for the refname, or we do not even have to have such a remote-tracking branch when this form is used). If <expect> is the empty string, then the named ref must not already exist.

Note that all forms other than `--force-with-lease=`<refname>`:`<expect> that specifies the expected current value of the ref explicitly are still experimental and their semantics may change as we gain experience with this feature.

`--no-force-with-lease` will cancel all the previous `--force-with-lease` on the command line.

A general note on safety: supplying this option without an expected value, i.e. as `--force-with-lease` or `--force-with-lease=`<refname> interacts very badly with anything that implicitly runs `git` `fetch` on the remote to be pushed to in the background, e.g. `git` `fetch` `origin` on your repository in a cronjob.

The protection it offers over `--force` is ensuring that subsequent changes your work wasn’t based on aren’t clobbered, but this is trivially defeated if some background process is updating refs in the background. We don’t have anything except the remote tracking info to go by as a heuristic for refs you’re expected to have seen & are willing to clobber.

Delete the branch with an expected tip

The final checks do not protect the deletion. If an authorized actor pushes an unmerged commit after the compare request and before gh_api("DELETE", ...), the GitHub REST endpoint deletes the branch ref at its new tip.

The delete-reference endpoint has no expected-SHA condition. Replace this call with an atomic lease-checked deletion, such as a Git protocol delete using --force-with-lease=<ref>:<scanned_sha>. A second GET cannot close this race.

REVIEW: PASS amw-qa dd4d69a

QA PASS at dd4d69a. Port of agent-middleware-api#829 (compared via gh): github-org-configuration runbook, README/doc link fixes, test_sqlite_failed_cursor_release.py. Local: gateway 5424 passed, 87 skipped; SDK 392 passed. Required CI green on this exact head (CI result, License result, gitleaks; Postgres proofs/concurrency pass). Merges cleanly.

REVIEW: FAIL deepseek dd4d69a

Findings:

 • .github/scripts/github_repo_settings.py:208: the protection PUT body carries only three required_pull_request_reviews subfields. Applying unrelated drift to an existing rule silently drops require_last_push_approval, dismissal_restrictions and bypass_pull_request_allowances, and _protection_view() doesn't report them. This can weaken branch protection without ever showing up as drift.
 • .github/scripts/github_branch_cleanup.py:~153-160: the tip and compare checks are separate requests, and the ref DELETE has no expected-SHA guard. A push that lands between the compare and the delete gets deleted at its new, unmerged tip. Use a lease-checked delete or refuse automatic deletion.
 • .github/scripts/github_repo_settings.py:247: check_secret_scanning treats every GhError from the PATCH as a secret-scanning entitlement refusal. Auth, 404, 422 and 5xx errors then exit 1 instead of the documented 2. Handle only the entitlement status and re-raise the rest.
 • .github/scripts/github_branch_cleanup.py:113: in --stale-days mode, the tip-commit lookup sits outside the per-branch try. One failed GET aborts the whole scan instead of skipping that branch.
 • gateway/docs/github-org-configuration.md:33: says CONTRIBUTING.md names License result, but CONTRIBUTING.md doesn't mention that check. The attribution is wrong.
 • gateway/app/main.py:576-577 / gateway/docs/openapi.json:7-8: the OpenAPI license name still reads "Business Source License 1.1", while the gateway is FSL-1.1-ALv2 (gateway/LICENSE.md). The public API metadata misstates the license.

`--apply` deletes branches selected by the script. It is a separate action
after review of the dry-run list. The script does not delete the archived

Correct the master-branch deletion claim.

.github/scripts/github_branch_cleanup.py:54-167 excludes only the base branch, protected branches, and configured keep prefixes. With --base main, --repo <repo> --apply can delete an unprotected merged master branch. Rewrite this sentence to scope it to the default repository, or add an explicit exclusion if master must never be deleted.

REVIEW: FAIL deepseek 17f1580

Safety check: both scripts are dry-run by default. Nothing is deleted or written without --apply (github_branch_cleanup.py prints "Dry run" unless --apply is passed; github_repo_settings.py gates its PATCH/PUT on apply). With --apply there's no further confirmation step, though, and these problems remain:

 • .github/scripts/github_branch_cleanup.py:147-160: under --apply, branch deletion is check-then-delete. The tip and compare are re-read, then DELETE git/refs/heads/
runs with no expected-SHA guard, so a push landing after the compare gets deleted at its new, unmerged tip. Use a lease-checked delete or an interactive confirmation.
 • .github/scripts/github_branch_cleanup.py:72: tuple(args.keep_prefix or DEFAULT_KEEP_PREFIXES) replaces the defaults. Passing any --keep-prefix silently drops the backup/ guard, so merged backup/* branches become eligible for deletion. Add the defaults back instead of replacing them.
 • .github/scripts/github_repo_settings.py:208 (PUT ~:220): the branch-protection PUT replaces the whole rule but carries only three required_pull_request_reviews fields. Live require_last_push_approval, dismissal_restrictions and bypass_pull_request_allowances get dropped whenever other drift triggers the PUT, which weakens protection without ever showing up as drift.
 • .github/scripts/github_repo_settings.py:245-255: every GhError from the secret-scanning PATCH is reported as an entitlement refusal and returns 1, not the documented 2 for gh failures. The message also cites a CI secret_scan job, but the job in .github/workflows/secrets.yml is gitleaks.
 • .github/scripts/github_branch_cleanup.py:113: in --stale-days mode, the tip lookup sits outside the per-branch try, so one failed GET aborts the whole scan.
 • gateway/docs/github-org-configuration.md:33: says CONTRIBUTING.md names License result, but it doesn't.

…refix, preserve protection fields, entitlement exit codes, resilient stale scan, runbook CONTRIBUTING claim

Passthrough rejects omitted review fields

Medium Severity

_review_passthrough treats a missing require_last_push_approval, dismissal_restrictions, or bypass_pull_request_allowances key, and a missing or null users/teams/apps list, as unrepresentable. GitHub omits those keys when the feature is off and often omits or nulls apps. Any drift on an existing review rule then raises before the PUT, so dry-run and --apply cannot update typical org repos.

Ref check breaks slashed branch deletes

Medium Severity

The new pre-delete GET git/ref/heads/{name} percent-encodes slashes in the branch name. GitHub’s git-ref GET treats %2F as a different path and often 404s for names like dependabot/... or feature/.... That error is treated as a hard failure, so --apply skips DELETE for the merged branches this script is most likely to target.

Allow absent optional review restrictions.

GitHub’s documented branch-protection response omits bypass_pull_request_allowances. If that field is absent and a managed setting drifts, _review_passthrough raises GhError before --apply can update the rule. Preserve and validate a restriction when GitHub returns it, but treat an absent optional restriction as unset. Add an absent-field case to the PUT test. (docs.github.com)

REVIEW: PASS deepseek 50d5ff9

Re-review of the six findings from the FAIL on 17f1580:

 1. Fixed. .github/scripts/github_repo_settings.py:143-179 rebuilds the review block from the live rule, keeping require_last_push_approval, dismissal_restrictions and bypass_pull_request_allowances, and merges it into the PUT at :250-252. Values it can't represent fail closed with exit 2.
 2. Fixed, with an acceptable window. .github/scripts/github_branch_cleanup.py:164-185 re-reads the tip, re-runs the compare, and re-reads the git/ref, then deletes only if the ref still equals the scanned SHA. The remaining ref-to-DELETE window is small and documented (:19-22, gateway/docs/github-org-configuration.md:53-55). Deletion still requires --apply, and the default is a dry run.
 3. Fixed. github_branch_cleanup.py:80 adds --keep-prefix to the defaults, so backup/ is always kept.
 4. Fixed. github_repo_settings.py:292-301 maps only 403/404 entitlement errors to exit 3 and re-raises everything else as exit 2. The doc now names the gitleaks job (github-org-configuration.md:47).
 5. Fixed. github_branch_cleanup.py:123-130 moves the stale-tip lookup inside the per-branch try, records scan failures, and exits nonzero.
 6. Fixed. github-org-configuration.md:36-39 attributes License result to license-check.yml.

No new blocking problems.
