fix(gateway): permit allowlist is checked before the tool registry (unknown tool → permit_tool_not_allowed) - #232
Product call from Lead: when a governed call carries an explicit permit, the permit's allowed_tools is the authority, so a tool the permit does not name is refused as out of scope whether or not it is registered.
Behavior
Explicit permit + tool not inallowed_tools + tool unregistered → -32003 permit_tool_not_allowed (JSON-RPC, /mcp/messages and standard /mcp) / HTTP 403{error: permit_tool_not_allowed} (REST), with details.requested_tool/allowed_tools and a signed denied receipt (reason_code permit_tool_not_allowed, zero credits, no ledger entry). The permit's own validation decides the code, so an earlier failure (wallet/key mismatch) keeps its narrower reason. Idempotent replay returns the same denial.
Explicit permit that names the unregistered tool → unchanged -32001 / 404.
Server-minted permits (standard /mcp): without an explicit permit, _governed_tools_call still returns -32001before minting, so an auto-mint never happens for an unknown tool (a minted permit always has allowed_tools == [tool] for a registered tool, so it cannot hit this branch). With an explicit _meta["io.agentmiddleware/permit_id"], the pre-mint local-registry short-circuit is skipped and the governed pipeline decides (it also consults persistent registrations, which the shortcut did not). Auto-minted calls for registered tools are unchanged.
Code: mcp.py new _deny_unregistered_tool_outside_permit called in the not service branch after durable replay and before "Tool not found"; mcp_standard.py only short-circuits -32001 when no permit was supplied.
Teststests/test_permit_unknown_tool.py (7): JSON-RPC and REST denial + receipt verified + audit + no debit; replay; permit-named unknown tool still not found; no-permit unchanged; standard /mcp explicit permit denial; standard /mcp auto-mint path unknown tool still -32001.
Local: full gateway suite 5796 passed, 87 skipped, 1 xfailed.
Note
High Risk
Changes core authorization ordering, permit signature semantics, and idempotency behavior on governed MCP paths—areas where mistakes could leak permit data, mis-charge, or strand or replay requests incorrectly.
Overview
Governed MCP invocation is reordered so permit scope, quotes, and idempotency run before tool registry resolution. A call with an explicit permit naming a tool outside allowed_tools now gets permit_tool_not_allowed (403 / -32003) with a signed denial receipt and replayable idempotency record, whether the tool is registered or not—so responses and audit metadata no longer leak registry state.
Security:PermitService.validate_for_action verifies the Ed25519 signature before reading allowed_tools, status, or other fields, closing probing of foreign or forged permits via allowlist echoes. verify_signature fails closed on malformed signed payloads.
Idempotency: Permitted calls that fail only because the tool is missing or not executable abandon the governed idempotency claim (_release_unresolved_tool_idempotency) instead of pinning a terminal replay, so a same-key retry can succeed after registration fixes. Action permits get parallel out-of-scope and binding-denial paths documented in failure-semantics.
Standard /mcp skips the early “tool not found” short-circuit when the client supplies an explicit permit id, deferring to the governed pipeline. Docs (POLICY_ENFORCEMENT.md, failure-semantics.md) and a large test_permit_unknown_tool.py suite cover parity, replay, and leak regressions; tests gain resign_permit_model after in-place permit edits.

Governed calls with an explicit permit now distinguish unregistered tools excluded by the permit from tools allowed by it. Excluded tools produce a permission denial, with denial auditing and a signed receipt when a permit, wallet, and idempotency key are available. Tools allowed by the permit continue to the existing not-found handling. Calls without an explicit permit retain the existing not-found behavior. Tests cover these outcomes across JSON-RPC, REST, and standard MCP calls.
Fix the misleading signed denial receipt and strengthen the no-key and action-permit tests before merging.
Security architecture risk: Low · up to f2f51
The change preserves wallet ownership checks and prevents unregistered tools from executing or charging credits. No newly demonstrated authorization bypass was found. Denial evidence and recovery behavior still have unresolved coverage, so the assessment is not minimal risk.
Retained concerns
No architecture-level concerns identified.
Security review details
Security Blast Radius
inferred — The demonstrated effects are gateway authorization responses, audit events, denial receipts and caller-scoped replay state. The new unregistered-tool branch does not grant downstream execution authority, charge another wallet, or expose permit allowlists after wallet/key mismatch.
Security Findings and Attack Paths
inferred — An authenticated caller using its own wallet can now distinguish a missing permit ID from an existing mismatched permit through an unregistered-tool response. Registered-tool validation already exposed the same distinction at the base. No newly expanded cross-tenant data access or execution privilege was demonstrated; the two supplied candidates remain deferred, not verified findings.
Trust Boundaries and Controls
observed — Caller-controlled wallet and tool inputs remain subject to wallet ownership and bounded-key checks before persistence. Permit identity checks gate allowlist-derived details. Replay separately enforces stable permit subject identity, with a narrow exception for the original requester's foreign-wallet denial receipt.
Resilience and Maintainability Implications
inferred — Terminal-evidence recovery remains incompletely established. Receipt insertion and idempotency completion are separate writes; the inspected recovery selectors do not establish repair of an uncharged unresolved-tool owner. Wrong-key denial replay also rejects access before returning the stored receipt. Related finalization and replay limitations predate this PR, so they are not retained as newly introduced security concerns.

Apply the allowlist check before rejecting a missing action binding.
If an explicit action permit names a registered action tool but the caller requests a different, unregistered tool, is_action is true and action_binding is absent. This branch returns action_tool_binding_required before the new allowlist check runs. On the newly forwarded /mcp path, the request therefore never receives the intended permit_tool_not_allowed reason. Preserve action-binding enforcement after deciding whether the permit names the requested tool.

Return before checking a foreign permit’s allowlist.
A foreign wallet that already knows a permit ID can probe unregistered tool names. The current branch returns different outcomes for allowlist members and non-members, exposing one bit per request. Permit reads are owner-, issuer-, or admin-scoped, and this path cannot execute or charge a tool, so the impact is a minor metadata disclosure rather than a major authorization bypass.

Check the allowlist before rejecting a missing action binding.
An action permit is issued for a registered action tool, but the invocation can request a different, unregistered tool. is_action remains true because it is derived from the persisted permit fields. The missing tool has no action_binding, so the router raises action_tool_binding_required before the allowlist branch can return permit_tool_not_allowed. Repeated requests also bypass governed idempotency.
Move the excluded-tool check before that binding rejection. Keep the existing binding behavior for a tool named by the action permit.

Merge resolution (_raise_replayed_error): correct. Reading details above the 403 branch (vs main's read inside it) is behaviorally equivalent: only the 403 ToolPermissionDenied branch uses it, with the same isinstance(details, dict) guard. The keyed replay test confirms the replay returns the same code, details and receipt.
The claimed happy path works. A plain permit plus an unknown tool returns -32003 permit_tool_not_allowed with a denial receipt, and permit-less calls are unchanged (-32001). No reservation, charge or dispatch happens on the new path.
Findings
Major: allowlist oracle before binding validation (gateway/app/routers/mcp.py_deny_unregistered_tool_outside_permit, ~L1228-1240). The helper reads allowed_tools_json and returns early ("Tool not found", -32001) when the name is on the permit. This happens before validate_for_action checks wallet, key and signature. A caller that presents another wallet's permit_id gets "Tool not found" when an unregistered name is on that permit, and permit_wallet_mismatch when it isn't. That leaks the foreign permit's scope, which main did not, since main always answered "Tool not found". Fix: run the permit's wallet/key/signature validation first, and only then branch on allowlist membership.
Minor: action permits don't get the new behavior (mcp.py ~L1419 vs ~L1478). For an action permit and an unregistered tool, action_tool_binding_required is raised before the new check, so the out-of-scope call never returns permit_tool_not_allowed and gets no receipt. Either document this exclusion in the PR or test it explicitly.
Minor: no-idempotency-key ordering (mcp.py ~L1271 vs ~L1649). A governed call with a permit, an unknown tool and no idempotency key now issues a signed denial receipt. A registered tool in the same situation is refused with idempotency_key_required first. Repeated keyless calls each mint a new denial receipt instead of a replayable outcome. Check the key requirement first so the two paths match.
Nit (real-data rule):gateway/tests/test_permit_unknown_tool.py uses invented tool names (permit-scope-allowed-tool, permit-scope-unregistered-tool, L36-37) and an invented {"message": "hello"} argument (~L191). An unknown-tool test needs some unregistered name. Use a data-free one, or one derived from the registry (e.g. a registered tool's name with a suffix), and empty or real arguments.
Finding 1 is a new information-disclosure regression on a security boundary, so the verdict is FAIL. The merge resolution itself is fine.
With an explicit permit, an unregistered tool that is not in allowed_tools now returns -32003 permit_tool_not_allowed (JSON-RPC, /mcp/messages and standard /mcp) or HTTP 403 (REST). The response carries a signed denied receipt and has zero charge. A tool the permit does name but the registry lacks still returns "Tool not found". This matches Lead's product call.
mcp_standard.py no longer auto-mints for an unregistered tool when no permit is given (still -32001), and the action_permit_required guard is now null-safe.
Tests: test_permit_unknown_tool.py covers the new path. Local: 6075 passed, 103 skipped. CI: all green, BEHIND main, mergeable.
Missing test (nice to have): a replay of the denied call with the same idempotency key, to show that details survive _raise_replayed_error. The diff hoists details for exactly that case.

Codex review of #232 at d14a2f0. Since 1babe40 the only change is a merge of main (#246, #247); the PR's own code is unchanged.
The requested behavior holds on the ordinary permit path. With a permit and an unregistered tool the permit does not name, the call returns -32003 with a signed denial receipt, and permit-less calls still return -32001. Every finding from the earlier 1babe40 FAIL is still open, though:
High: allowlist oracle (gateway/app/routers/mcp.py:1228-1237, _deny_unregistered_tool_outside_permit). The helper loads the permit and returns early ("Tool not found", -32001) when tool_name is in allowed_tools_json. That happens before validate_for_action checks wallet, key and signature (L1238). A caller holding another wallet's permit_id can tell allowed names (-32001) from disallowed ones (a denial). Run validate_for_action (or at least the wallet/key binding) before reading the allowlist, or return the same refusal for both cases when the binding fails.
Medium: action permits never reach the new path. action_tool_binding_required is raised first (mcp.py:~1419), so there is no permit_tool_not_allowed and no receipt. Document this exclusion or test it.
Medium: with no idempotency key, the new denial (mcp.py:~1478) mints a receipt before idempotency_key_required. A registered tool would be refused first, so each repeated keyless call mints a new receipt.
Medium (real-data rule):gateway/tests/test_permit_unknown_tool.py:36-37 and ~L191 commit invented tool names and arguments. Derive the unknown name from the registry at runtime (for example a registered name plus a uuid4() suffix) and use empty args.
Delta since 1babe40: a plain merge of main (be3d81f), and the remerge-diff is empty.
pytest: 6075 passed, 103 skipped
Open CodeRabbit thread (mcp.py:1484), not addressed: under an action permit, an unnamed tool is refused with action_tool_binding_required before the allowlist branch returns permit_tool_not_allowed. The call is still refused, but the code differs and governed idempotency is skipped. Follow-up recommended.
dsh headless review of the full PR diff (merge-base..d14a2f0), run Oct 26 at 6:54 AM PT. This supersedes the earlier hand review for this head.
Review
I traced all three entry points (/mcp/messages, /mcp, and the legacy REST invoke) through _execute_registered_tool_inner, inspected the replay path, and ran the new suite plus the existing trust, idempotency, denial-detail, auto-permit, and standard-MCP tests. They pass. The no-permit path and the auto-mint path are unchanged. The issues below are in the new permitted-denial path.
1. The new path still leaks tool registration through caller-readable audit metadata
gateway/app/routers/mcp.py:1249-1255 (decision built with estimated_cost=None)
The registered out-of-scope denial does not write tool_registered at all and records the real cost:
gateway/app/routers/mcp.py:1609-1615
gateway/app/routers/mcp.py:1786-1798
Audit metadata is returned verbatim to the wallet:
gateway/app/routers/me.py:256-299
gateway/app/routers/audit.py:49-97
gateway/app/schemas/audit.py:22
gateway/app/services/audit_log.py:53-75
A wallet-scoped caller can therefore call an out-of-permit tool and then read its own /v1/me/audit/events entry. tool_registered: False (and estimated_cost: None) distinguishes an unregistered tool from a registered one, which is exactly the leak this PR is trying to close.
2. The signed denial receipt also distinguishes registered from unregistered tools
gateway/app/routers/mcp.py:1285 passes registered_cost=Decimal("0") for the new path, while the registered out-of-scope denial passes the real tool cost at gateway/app/routers/mcp.py:1817.
That value becomes the receipt field credits_authorized (gateway/app/services/receipts.py:469, gateway/app/schemas/trust.py:319), and the full receipt is returned in the error response. So a caller sees credits_authorized == 0 for an unregistered tool and a nonzero cost for a registered tool. Removing only tool_registered from the audit metadata would not fix the leak.
3. The new check runs before the idempotency-key and quote gates, so errors still differ by registration
The new denial is called inside the if not service block at gateway/app/routers/mcp.py:1478-1500, before:
the governed idempotency_key_required check at gateway/app/routers/mcp.py:1649-1662
the quote check at gateway/app/routers/mcp.py:1571-1604
Consequences:
A permit-bearing request with no idempotency key returns 403 permit_tool_not_allowed for an unregistered tool, but 403 idempotency_key_required for a registered tool.
A request with a bogus quote_id returns 403 permit_tool_not_allowed for an unregistered tool, but 403 quote_not_found for a registered tool.
With no idempotency key, the new path creates a denial receipt but no terminal idempotency record (_finalize_governed_denial falls back to idem.complete(..., key="")), which is a different contract from the registered path.
Both are additional existence oracles and inconsistent error contracts.
4. The tests do not assert the no-leak property
gateway/tests/test_permit_unknown_tool.py:67-75 never checks credits_authorized.
The audit assertion at gateway/tests/test_permit_unknown_tool.py:111-115 uses _assert_denial_audited (gateway/tests/test_mcp_trust_mode.py:106-124), which only checks reason, transport, and request hash, not tool_registered or estimated_cost.
No test compares a registered out-of-scope denial with an unregistered one, so the parity that motivated the PR is untested.
_assert_no_tool_debits at gateway/tests/test_permit_unknown_tool.py:116-121 filters ledger entries by the unregistered tool name, so it is vacuous for this case.
5. Minor
idem_begin is passed to _deny_unregistered_tool_outside_permit but never used (gateway/app/routers/mcp.py:1203), suggesting the record linkage was intended but dropped.
assert record is not None in gateway/app/routers/mcp_standard.py:511 is a production assert; the guard above makes it safe, but it is not a control-flow guarantee under python -O.
gateway/docs/failure-semantics.md:57-63 still says unknown-tool rejections produce no receipt, which now contradicts the new permitted-denial behavior.
…ed tools (#232)
- Run validate_for_action first; a foreign or invalid permit denies with
its own reason and no allowlist details (no cross-wallet tool oracle).
- Action permits naming an unregistered tool get the same permit decision
instead of action_tool_binding_required.
- Mint a denial receipt only when the call carries an idempotency key.
- Tests use real registered tool ids (partner.notes.write / .count).

Do not issue a permit receipt for a foreign-wallet mismatch.
When permit_wallet_mismatch is returned, the helper passes the foreign permit_id and caller wallet_id to create_receipt. The signed receipt is accepted by verify_receipt and is accessible to the caller as its wallet’s receipt, although the caller does not own the permit. Use the existing idempotency-only denial path for this mismatch. This preserves the denial reason without creating false permit ownership evidence.

Use a valid action permit to test the action denial.
action_runtime is defined only in test_action_invocation.py and is not registered as a pytest plugin, so this test cannot reuse it directly. Reproduce its small local setup: register BINDING, provision the wallets and key, issue /v1/action-permits, and use the returned permit ID. Then assert permit_tool_not_allowed exactly.

Blocking (P1): the signature is still checked after allowed_tools is read. validate_for_action checks wallet/key at gateway/app/services/permits.py:980, reads and can return allowed_tools at permits.py:996, and verifies the signature only at permits.py:1112. A badly signed, out-of-scope permit can still reveal its allowlist and reach the denial-receipt path (called from gateway/app/routers/mcp.py:1237; the action path at mcp.py:1420 shares the helper). Please move signature verification ahead of the allowlist read and add a bad-signature test.
Verified:
A foreign wallet's permit gets permit_wallet_mismatch before the allowlist, and details are suppressed (permits.py:980, mcp.py:1248).
Unregistered action tools go through the same helper (mcp.py:1420).
A receipt is finalized only when an idempotency key exists (mcp.py:1271).
The tool ids partner.notes.write and partner.notes.count are real (dogfood_tool.py:31).
All 3 new tests would fail on d14a2f0 (old mcp.py:1232-1237, 1271, 1421).
CI is green apart from the CodeRabbit check, which is still pending. The branch is behind main.
Non-blocking:
gateway/tests/test_permit_unknown_tool.py hardcodes request and idempotency ids (lines 111-432). Prefer uuid4.
The no-key test (around line 407) accepts any error code except -32003. Assert the expected code.
Pytest was not run because the review ran read-only.
validate_for_action now checks the permit signature right after the wallet and key binding, before allowed_tools, scopes or budgets are read or echoed. That covers the tool path and the action path. verify_signature fails closed when the signed payload cannot be rebuilt. Tests: forged-signature cases (tool and action) fail on the old order, uuid4 request and idempotency ids, and an exact error code in the no-key test.

Move the signature check in validate_for_action ahead of the status,
expiry and wallet/key binding checks, not just ahead of allowed_tools, so
a forged row is answered only with permit_signature_invalid and never
reaches the denial-receipt paths. New test: a forged row also marked
revoked still gets permit_signature_invalid (fails on the previous order).
Tests that edit signed fields in place (backdated expiry, planted limits,
emptied allowlist) now re-sign the row through a shared helper, and the
REST test uses a uuid4 idempotency key.

My earlier blocker is fixed. In _validate_model_for_action, verify_signature now runs right after the credit-amount check (permits.py:960), ahead of the status, expiry, wallet/key binding and allowed_tools reads (allowed_tools is read at :1008). A forged row only gets permit_signature_invalid, and the new forged-plus-revoked test covers that order. The tests that edit signed fields now re-sign through the shared helper in test_trust_helpers.py, and I found no weakened assertions or other regressions in the delta. CI is green at this head.
Codex also flagged two gaps that exist unchanged on main and sit outside this PR's diff, so I'm treating them as follow-ups rather than blockers here:
validate_replay_access (permits.py:632) compares the wallet and key bindings before verify_signature. It fails closed with a bare reason code and echoes no fields, but the signature should still come first.
reserve_budget (permits.py:1244) reloads the permit under lock and trusts its bindings and budgets without re-verifying the signature.
A non-blocking note: the refreshed denial fields around permits.py:815 aren't re-verified. Postgres row locks cover that path, but SQLite race behavior is unverified. The database-backed suite wasn't run locally because the review was read-only. CI ran it.

Registered-but-not-allowed and unregistered tools now share one denial path, _deny_tool_outside_permit, before the Tool not found check. Each item has a test that fails on d14a2f0 and passes at head:
credits_authorized=0 receipt (~L1285): denial receipt minted with registered cost 0 either way. Test: test_item2_denial_receipt_identical_for_registry_state.
Gate ordering (~L1478-1578): idempotency_key_required and quote validation decided before the permit-scope denial, independent of registration. Test: test_item3_same_gate_fires_first_for_registry_state[no-idempotency-key|bogus-quote].
Also test_denials_do_not_reveal_registration (REST + JSON-RPC). Full suite: 6178 passed, 103 skipped. Head adds tests only; no code change.

Delta review e27b2a6..0938d76 (one commit, tests only: gateway/tests/test_permit_unknown_tool.py +116). e27b2a6's main merge remerge-diff is clean.
The new per-item registration-parity tests do exercise both registry states for audit metadata, denial receipts and gate ordering. Tool-path signature-first ordering from e83c5e5 is intact.
Blocker (data-free rule): test_permit_unknown_tool.py:761 adds the hard-coded id "quote-does-not-exist" in the bogus-quote parametrize case. Please generate it at runtime, e.g. f"quote-{uuid4()}".
Non-blocking, pre-existing on main (not from this delta): on the action-permit path, mcp.py:1401 / action_permits.py:410 check binding/status/expiry before signature verification. Suggest a separate ticket.

Made-up quote ids (and the registration-parity request ids) in gateway/tests/test_permit_unknown_tool.py are now generated at runtime with uuid4(); no other hard-coded ids remained. Test-only change; test file 25 passed, full suite green.
Delta re-review from my FAIL at d14a2f0. I ran a real dsh headless review on 0938d76 (Oct 26, 9:50 PM PT). The head then moved to 916ce1c, a test-only change in test_permit_unknown_tool.py that generates the quote and request IDs at runtime. I checked that diff by hand and re-ran the tests on it. None of the findings below change.
Prior findings: all three leaks fixed
Audit metadata: tool_registered is gone, and estimated_cost is left out on every ok=False event (mcp.py:1247-1259, :4612-4616). Registered and unregistered out-of-scope denials now share _deny_tool_outside_permit (mcp.py:1559-1577).
Receipt: both registry states get credits_authorized == 0 (mcp.py:1264-1283). test_item2 asserts it.
Gate order: the idempotency-key and quote gates now run before the permit-scope check (mcp.py:1456-1529). test_item3 asserts it.
Proof tests: the head's test_permit_unknown_tool.py passes 25/25 at 0938d76 and again at 916ce1c. Run against d14a2f0 code, 18 fail and 7 pass. The test_item1/2/3 failures are the expected assertion failures: tool_registered present, credits_authorized 1 vs 0, and permit_tool_not_allowed vs idempotency_key_required/quote_not_found. The related suites (permit*, mcp*, trust*, denial*, idempot*) pass: 896 passed, 36 skipped.
Merges: the main merges e27b2a6, dedab8f and fd98b6d have empty remerge-diffs. e83c5e5, a branch-into-branch merge, resolves its conflict in the test file correctly.
Blocking
Permit-bearing calls to a non-executable tool now mark their idempotency key terminal. The governed begin runs before the executable checks whenever permit_id is set (mcp.py:1499-1529). _complete_governed_denial_idempotency then stores a terminal 400 Tool not executable (mcp.py:1595-1630). After the registration is fixed, a same-key retry replays the old failure instead of executing. At d14a2f0 nothing was recorded. This contradicts failure-semantics.md:64-69. Either move the begin below the executable checks, or change the contract and add a test for it.
test_action_permit_unregistered_tool_gets_permit_decision passes for the wrong reason. It sets action_contract_version = 1 on a permit that is already signed (test_permit_unknown_tool.py:560-564). The response is permit_signature_invalid, so the action permit-scope path never runs. Its no-receipt assertion would also be wrong for a valid action permit, because _action_request_denial_receipt mints one (mcp.py:1006-1080). Use a real action permit from create_action_permit instead.
Docs are stale. failure-semantics.md was not updated. Lines 57-63 still say an unknown tool gets Tool not found with no receipt. At head, an unregistered tool outside a valid permit gets 403 permit_tool_not_allowed with a signed receipt. Lines 64-69 are now wrong too (see item 1). POLICY_ENFORCEMENT.md:48-52 and :95 also show the old ordering, with the signature checked last, but permits.py:952-965 now checks the signature first.
Non-blocking
Idempotency records still differ by registration. operation_kind is unresolved vs the backend name, and wait_timeout_seconds is 0 for upstream tools (mcp.py:1505-1524). Neither is readable by the caller today. But under a concurrent same-key request, the difference can show up as idempotency_in_progress vs a replayed denial.
A production assert record is not None remains at mcp_standard.py:511.
CI at review time: lint, CodeQL, gitleaks, Postgres concurrency and Framework integrations pass. The Python 3.11-3.14 and Postgres proofs jobs were still pending when I posted, so they aren't counted.
A registry-state failure is no longer stored as the key's final answer, so a
same-key retry executes once the tool works. Fix the action-permit test to use
a validly signed action binding, update failure-semantics and
POLICY_ENFORCEMENT ordering, and replace a production assert.

Idempotency key made terminal on a non-executable tool. Fix: new _release_unresolved_tool_idempotency in gateway/app/routers/mcp.py. On a permit-bearing call, the three Tool not executable sites and Tool not found now abandon the pinned governed claim (the record id is checked, and nothing ran or was charged) instead of storing a terminal 400. Permitless Tool not found keeps its documented terminal record (test_strict_mode_replays_missing_tool_lookup_error_with_idempotency_key is unchanged). Test: tests/test_permit_unknown_tool.py::test_unresolved_tool_failure_is_not_replayed_after_fix[not_executable|not_registered]. It calls while the tool can't run, then retries with the same key after the tool is fixed, and the retry must execute. Both cases fail on 916ce1c, where the retry replays Tool not executable / Tool not found.
The action-permit test passed for the wrong reason.test_action_permit_unregistered_tool_gets_permit_decision now sets a complete action binding and re-signs it with resign_permit_model. It asserts exactly permit_tool_not_allowed, not permit_signature_invalid, and checks the signed denial receipt that a valid action permit mints. This is a test-only fix, so it passes on both heads.
Stale docs.gateway/docs/failure-semantics.md covers the 403 permit_tool_not_allowed and its receipt for out-of-scope tools whether or not they are registered, and the released claim for unresolved tools on permitted calls. gateway/docs/POLICY_ENFORCEMENT.md now shows the real pipeline order (idempotency-key and quote gates, then idempotency begin, then permit scope, then tool resolution) and the permit signature checked first.
Non-blocking
mcp_standard.py:511: the assert is replaced with an explicit if record is None: raise RuntimeError(...).
operation_kind / wait_timeout_seconds parity: not changed. operation_kind="upstream_mcp" drives the reconciler (idempotency.py ~:61, :983, :1037-1150), so unifying it isn't safe in this PR. Because item 1 now releases the unresolved record, the window where the difference could show is limited to a concurrent same-key request during one call.
origin/main is merged in plainly (460722c); its remerge-diff is empty. Full suite: 6180 passed, 104 skipped. Ruff is clean.

Verified
Releasing the claim (claim 1):test_unresolved_tool_failure_is_not_replayed_after_fix[not_executable|not_registered] fails on 916ce1c. That code replays the stored "Tool not …" error. Both cases pass at head. _release_unresolved_tool_idempotency pins expected_record_id (mcp.py:3868-3873), and idem.abandon won't release a row that already has a response or ledger entry (idempotency.py:850-864). Nothing before the release writes data, so I found no way to double-run the call on the non-action path. Strict-mode calls without a permit still save the final "Tool not found" record (mcp.py:1590-1600), and test_mcp_trust_mode.py:402-447 passes.
Action-permit test (claim 2): it re-signs the permit with resign_permit_model, and for the unregistered tool it really returns permit_tool_not_allowed with a valid signed receipt.
Claim 4:mcp_standard.py:511 now raises RuntimeError. The operation_kind dependency is real. reconcile_stuck_records runs from main.py:382 and sweeps only rows where operation_kind == "upstream_mcp" (idempotency.py:983-985). may_abandon_action_owner checks that value too (idempotency.py:61).
Main merge (claim 5):460722c has an empty remerge-diff.
Tests:test_permit_unknown_tool.py 27/27. The related suites (permit, mcp, trust, denial, idempotency) give 898 passed, 37 skipped. The production_trust run gives 12 passed.
Blocking
With an action permit, registered tools still skip the permit-scope check, so the response reveals registration.mcp.py:1412-1435 only calls _deny_tool_outside_permit when not service. A registered tool with no action binding goes straight to raise ToolPermissionDenied("action_tool_binding_required") at :1435. A tool that has a binding runs validate_action_request (:1440) before allowed_tools is checked. The dsh run reproduced it: with an action permit for ALLOWED, a registered local tool outside the permit returns action_tool_binding_required, while an unregistered tool returns permit_tool_not_allowed. That contradicts failure-semantics.md:61-63 and POLICY_ENFORCEMENT.md:51, which say the response is the same for registered and unregistered tools. The new test (test_permit_unknown_tool.py:560-600) covers only the unregistered case.
Action-permit scope denials don't use idempotency.mcp.py:1419-1429 passes idempotency_key=None, and _action_request_denial_receipt mints a receipt with idempotency_record_id=None (mcp.py:1128-1142). Two calls with the same key produced two different signed receipts. That contradicts failure-semantics.md:63-66, which says the record is final and a replay returns the same response. No test makes a second call with the same key.
The docs and comments overstate how action permits behave.
failure-semantics.md:71-75 says a valid permit that names the tool gets the "Tool not found"/"Tool not executable" response and its claim released. With an action permit, the call hits action_tool_binding_required at :1435 first.
The comment at mcp.py:1416-1418 says the denial "carries no receipt", but the wrapper mints one.
Non-blocking
If the process crashes between saving the claim and releasing it, the row stays behind with operation_kind set to unresolved or local. reconcile_stuck_records never sweeps those rows (idempotency.py:983-985, :1089-1122), so a retry with the same key can get idempotency_in_progress forever. This was already true before this PR, but it makes the unqualified retry promise in failure-semantics.md:74-75 inaccurate.
The rewritten failure-semantics.md no longer says that a call without a permit to a registered but non-executable tool records nothing. test_mcp_trust_mode.py:451-499 still asserts that.
Delta from our PASS at d14a2f0, with own commits 0938d76, 916ce1c, f0916d6 and 468a36f plus merges of main. Gateway tests exit 0 and CI is 21 pass, 0 fail.
Blocking: an unresolved Cursor Bugbot Medium thread at gateway/app/routers/mcp.py:1434. The action-permit short-circuit calls _deny_tool_outside_permit only when service is missing. A registered tool without an action binding still returns action_tool_binding_required, so the error code reveals whether a tool name is registered. That breaks this PR's goal (unknown tool should give permit_tool_not_allowed). Fix it and add a test.

Out-of-scope action permits now take the governed tool-permit path before
any registry-dependent action branch: same code, audit and signed receipt
for registered and unregistered tools, and the real idempotency key so a

Action permits revealed registration. An action permit that does not name the tool (action_out_of_scope) now skips the registry-dependent action branch and takes the governed tool-permit path: idempotency begin, then _deny_tool_outside_permit (signature first). A registered tool with no action binding, and an unregistered one, both get permit_tool_not_allowed with identical audit and receipt. Partial action bindings are still validated before any binding decision, independent of the registry, so a tampered row returns permit_signature_invalid with no receipt. Test: test_action_permit_denial_identical_for_registry_state (fails on 468a36f: action_tool_binding_required). This also covers the QA item and the Bugbot thread (now resolved).
idempotency_key=None on scope denials. The real key is now used, and the denial completes a terminal record, so a same-key retry replays the first signed receipt. Test: test_action_permit_denial_replays_same_receipt_for_same_key (fails on 468a36f: idempotency_record_id is None).
Docs/comment. The mcp.py comment and failure-semantics.md now describe the action-permit path, including the minted receipt.
Non-blocking: I added the crash-between-claim-and-release note. The reconciler sweeps only upstream_mcp rows, so a local/unresolved row stays idempotency_in_progress until an operator clears it.
I restored the line that a permitless call to a non-executable tool records nothing.
Full suite: 6188 passed, 104 skipped, 1 failed. The failure is test_docs_reference_integrity, and it comes from main (#265's backup-restore-runbook.md references schema-0NN-rollout.md), not from this PR.

Action permit allowlist leaks before validation
High Severity
An action permit's allowed_tools is read before signature or wallet checks and selects the denial path. A complete in-scope permit with no action binding returns action_tool_binding_required without validating the caller, while an out-of-scope permit returns a validated permit_wallet_mismatch or permit_signature_invalid. Those two errors reveal whether the permit names the tool.
Delta review 468a36f..dd2ddb2 (fe7671c plus a merge of main; remerge-diff is empty).
Verified
Prior QA leak fixed: an action permit that does not name the tool now takes the governed _deny_tool_outside_permit path before any binding lookup, so registered and unregistered tools both get permit_tool_not_allowed (-32003), the same audit event and a signed receipt.
New tests test_action_permit_denial_identical_for_registry_state and test_action_permit_denial_replays_same_receipt_for_same_key fail with 468a36f app code and pass at head (tests/test_permit_unknown_tool.py: 30/30 at head, 28/30 with 468a36f app).
DeepSeek 5662554086 blockers: (1) the registry-dependent branch for out-of-scope action permits is gone; (2) the real idempotency key is used and a same-key retry replays the first receipt (tested); (3) failure-semantics.md, POLICY_ENFORCEMENT.md and the mcp.py comment are corrected.
Gateway pytest -q -p no:randomly: 6202 passed, 1 failed, 90 skipped. The failure is test_docs_reference_integrity (backup-restore-runbook.md -> schema-0NN-rollout.md), which arrived from main via #265, not from this PR.
Blocking
An in-scope action permit for a tool with no action binding is answered before the permit is validated (mcp.py ~1460, the raise ToolPermissionDenied("action_tool_binding_required") branch). allowed_tools is read from the unverified row and selects the path: in scope returns action_tool_binding_required with no signature or wallet check, out of scope returns a validated permit_wallet_mismatch or permit_signature_invalid. A caller holding another wallet's permit id, or a tampered row, can learn which tools the permit names. This is the open Cursor Bugbot thread (High) and it is valid. Fix: validate the permit (signature first, then wallet) before the binding decision for complete in-scope rows too, and add a test that a wrong-wallet or bad-signature permit gets the same error whether or not it names the tool.

Claim 1, for a valid permit owned by the calling wallet:test_action_permit_denial_identical_for_registry_state fails at 468a36f, which returns action_tool_binding_required. It passes at head. I also ran my own probe and compared the full JSON-RPC response, headers, audit row and receipt for the registered and unregistered cases. All 56 fields match except the expected per-call values: receipt/audit/idempotency-record IDs, created_at, signature, the idempotency key and the rate-limit headers.
Claim 2:test_action_permit_denial_replays_same_receipt_for_same_key fails at 468a36f, where idempotency_record_id is None. It passes at head. The second call replays the first receipt and mints no new one.
Claim 3: the "carries no receipt" comment is gone. failure-semantics.md:82-97 now covers both of my earlier notes: a call without a permit to a non-executable tool records nothing, and the crash window.
Claim 5:test_docs_reference_integrity.py::test_docs_markdown_references_exist fails identically on origin/main 2674423. The cause is backup-restore-runbook.md: schema-0NN-rollout.md from #265. It is the only failure at head: locally 6176 passed, 104 skipped, 1 failed, and the 3.11-3.14 CI jobs show the same single failure. production_trust: 12 passed. test_permit_unknown_tool.py: 30/30.
Blocking
The new action_out_of_scope gate reads the permit's allowed_tools before validating the permit (mcp.py:1415-1423). That membership test decides which path runs. Through a permit the caller doesn't own, it shows whether a tool is on the allowlist, which is the probe _deny_tool_outside_permit exists to prevent (mcp.py:1222-1226). The dsh run reproduced these, all with a permit owned by another wallet:
The out-of-scope call returns permit_wallet_mismatch with a signed receipt. The in-scope unregistered call returns action_tool_binding_required with no receipt, because the binding is checked before validation (mcp.py:1456-1461).
For the action-bound partner.pay, the out-of-scope call returns permit_wallet_mismatch with a receipt, and the in-scope call returns it without one. At 468a36f both returned no receipt, so this difference is new.
With a quote_id, the in-scope call returns action_quote_unsupported (mcp.py:1464-1465) and the out-of-scope call returns quote_not_found (mcp.py:1501-1523). At 468a36f both returned action_quote_unsupported, so this difference is new too.
I confirmed in the code that the membership test runs before any validation. The foreign-permit cases are dsh's reproductions, and I didn't rerun them separately. The new tests only use a valid permit owned by the caller. One more edge case: the binding lookup at mcp.py:1396 still runs before the scope check and can raise action_binding_registry_mismatch (service_registry.py:462-472). Fix: validate the permit (signature, then wallet and key binding) before reading allowed_tools. Then add tests with a foreign, an expired and a forged action permit.
The docs don't match these paths.
POLICY_ENFORCEMENT.md:52 says a bound tool goes to validate_action_request, but action_quote_unsupported fires first when a quote is supplied (mcp.py:1464-1465).
failure-semantics.md:74-76 says the binding-denial receipt is minted per call. mcp.py:1082-1092 mints none for a foreign subject or key.

failure-semantics.md:93 lists only unresolved and local for the crash window. A registered metadata_only tool produces metadata_only, which isn't swept either. upstream_mcp is swept.

Product call from Lead: when a governed call carries an explicit permit, the permit's
allowed_toolsis the authority, so a tool the permit does not name is refused as out of scope whether or not it is registered.Behavior
allowed_tools+ tool unregistered →-32003 permit_tool_not_allowed(JSON-RPC,/mcp/messagesand standard/mcp) / HTTP 403{error: permit_tool_not_allowed}(REST), withdetails.requested_tool/allowed_toolsand a signed denied receipt (reason_codepermit_tool_not_allowed, zero credits, no ledger entry). The permit's own validation decides the code, so an earlier failure (wallet/key mismatch) keeps its narrower reason. Idempotent replay returns the same denial.-32001/ 404.-32001/ 404.Server-minted permits (standard
/mcp): without an explicit permit,_governed_tools_callstill returns-32001before minting, so an auto-mint never happens for an unknown tool (a minted permit always hasallowed_tools == [tool]for a registered tool, so it cannot hit this branch). With an explicit_meta["io.agentmiddleware/permit_id"], the pre-mint local-registry short-circuit is skipped and the governed pipeline decides (it also consults persistent registrations, which the shortcut did not). Auto-minted calls for registered tools are unchanged.Code:
mcp.pynew_deny_unregistered_tool_outside_permitcalled in thenot servicebranch after durable replay and before "Tool not found";mcp_standard.pyonly short-circuits-32001when no permit was supplied.Tests
tests/test_permit_unknown_tool.py(7): JSON-RPC and REST denial + receipt verified + audit + no debit; replay; permit-named unknown tool still not found; no-permit unchanged; standard/mcpexplicit permit denial; standard/mcpauto-mint path unknown tool still-32001.Local: full gateway suite 5796 passed, 87 skipped, 1 xfailed.
Note
High Risk
Changes core authorization ordering, permit signature semantics, and idempotency behavior on governed MCP paths—areas where mistakes could leak permit data, mis-charge, or strand or replay requests incorrectly.
Overview
Governed MCP invocation is reordered so permit scope, quotes, and idempotency run before tool registry resolution. A call with an explicit permit naming a tool outside
allowed_toolsnow getspermit_tool_not_allowed(403 /-32003) with a signed denial receipt and replayable idempotency record, whether the tool is registered or not—so responses and audit metadata no longer leak registry state.Security:
PermitService.validate_for_actionverifies the Ed25519 signature before readingallowed_tools, status, or other fields, closing probing of foreign or forged permits via allowlist echoes.verify_signaturefails closed on malformed signed payloads.Idempotency: Permitted calls that fail only because the tool is missing or not executable abandon the governed idempotency claim (
_release_unresolved_tool_idempotency) instead of pinning a terminal replay, so a same-key retry can succeed after registration fixes. Action permits get parallel out-of-scope and binding-denial paths documented in failure-semantics.Standard
/mcpskips the early “tool not found” short-circuit when the client supplies an explicit permit id, deferring to the governed pipeline. Docs (POLICY_ENFORCEMENT.md,failure-semantics.md) and a largetest_permit_unknown_tool.pysuite cover parity, replay, and leak regressions; tests gainresign_permit_modelafter in-place permit edits.Reviewed by Cursor Bugbot for commit dd2ddb2.
