[audit-high] Audit verify missed sequence holes, a deleted tail, and a missing head - #28
The audit chain could be reported as valid after the tail event was deleted, as long as the verify call included a time window. Sequence numbers could be skipped or repeated and the chain still verified, because those numbers are not part of the signature and only the last number was compared to the head. Deleting the head row had the same result, and the next recorded event then started a second chain. A blank wallet id was stored on the same head as wallet-less events but checked as a different chain, so a real pair of events looked truncated.
Why it matters
Verify is the check operators use to trust the audit history. A valid result in these cases means a missing or reordered record can pass.
How it was fixed
Every verify loads the head with the events, then applies any time window only to the hash walk.
Sequences must be 1 through the head's last sequence, with no holes or duplicates (audit_sequence_gap).
A modern chain with events and no head fails verify (audit_chain_head_missing), and append refuses to sign a new genesis over those rows.
Blank and null wallet ids are verified as one chain, matching the shared head key.
Old rows that are still sequence 0 and have no head still verify.
Tests
Added gateway/tests/test_audit_chain_tamper.py. Before the source change, ~/metacode/fleet/heavy .venv/bin/pytest -q tests/test_audit_chain_tamper.py --tb=line -p no:cacheprovider (from gateway/) returned 8 failed, 3 passed in 1.74s. The failures were the new gap, duplicate sequence, time window, missing head, append, and blank wallet cases. After the fix, ruff check on the two changed files reported All checks passed!, and ~/metacode/fleet/heavy .venv/bin/pytest -q tests/test_audit_chain_tamper.py tests/test_audit_chain.py tests/test_audit_snapshot.py tests/test_audit_chain_contention_surface.py --tb=short -p no:cacheprovider returned 35 passed in 13.44s. A follow-up run of the new file alone returned 11 passed in 0.44s.
Notes for the reviewer
Pointing the head back at a shorter contiguous prefix (delete the tail and update last_seq plus last_chain_hash together) still verifies. That state matches a chain that never grew. Closing it needs an anchor outside this database.
AuditChainHeadMissingError is not contention. It is not retried. The next append on a leftover seq 0 chain with no head now raises instead of forking that chain.
No migration. No API field changes. A successful time-window verify still reports how many events were inside the window.
Draft PR from the Muse Code QA fleet (instance brk-gw-audit), reviewed by Grok Bot. Not merged or deployed; needs C.Lee's yes.
Note
High Risk
Changes the operator-facing audit integrity gate and append failure modes; missing-head handling can block new audit writes on damaged modern chains until the head is repaired.
Overview Tightens audit chain verification and append so tampered or incomplete chains no longer report valid=True.
Verification now always snapshots events with the chain head, then applies time windows only to which events count in the result—wide windows can no longer hide a deleted tail (audit_chain_truncated). The hash walk still covers the full chain so links outside the window are checked. After the walk, the head must match the true tail, sequences must be contiguous through last_seq (audit_sequence_gap), and a modern chain with events but no head row fails (audit_chain_head_missing). NULL and blank wallet_id are verified as one wallet-less chain via _chain_wallet_filter, matching the shared head key.
Append raises AuditChainHeadMissingError instead of signing a new genesis when seq-signed events exist without a head; legacy all-seq 0 chains without a head still verify and can append.
Adds gateway/tests/test_audit_chain_tamper.py and adjusts windowed-verify tamper expectations in test_audit_chain.py. BREAK-REPORT.md documents the four fixed breaks.

Walkthrough
Audit-chain signing and appending now treat NULL and blank wallet IDs as one chain. Verification loads the full chain and checks for missing heads, truncated tails, and sequence gaps, including when time bounds are set. If events exist without a head, appending retries once and then raises AuditChainHeadMissingError. The pull request adds tamper tests and a report of the changes, test results, and remaining limitations.
Verification of a valid legacy chain may incorrectly report tampering when a time window excludes its first event. This is a bounded compatibility risk to resolve or explicitly accept before merging.
Security architecture risk: Moderate · up to 377c5
The change strengthens tamper detection and preserves wallet authorization. The main remaining risk is rollout compatibility: existing headless legacy histories still verify, but further audit writes stop until their heads are repaired. The affected production population and recovery process are not established.
Retained concerns
Medium · reliability · inferred: Previously accepted legacy headless histories remain valid for verification but become unwritable after this change. The existing migrations and startup checks do not establish heads for those histories. If such data exists, audit-producing operations can fail, or existing best-effort callers can continue without the expected durable audit record, until an operator repairs the head. Rejecting a new genesis is the correct integrity protection; the unresolved issue is the transition and recovery process for legitimate legacy state.
Security review details
Security Blast Radius
inferred — Append contention and missing-head rejection are scoped to the affected wallet head, including the shared wallet-less head. A principal with write authority over both audit tables can affect every chain covered by those database permissions; per-wallet head keys are concurrency boundaries, not protection against a hostile datastore writer.
Security Findings and Attack Paths
inferred — A database writer able to delete a tail and rewind its head to a surviving contiguous prefix can still make that prefix appear complete without forging signatures. This limitation predates the PR and is explicitly documented; it is not an introduced architecture concern.
Trust Boundaries and Controls
observed — The verification route authenticates callers, requires exact wallet access for a requested wallet, defaults ordinary callers to their authenticated wallet, and requires bootstrap-admin access otherwise. Blank/NULL normalization reconciles an existing wallet-less head identity rather than combining concrete tenant wallet IDs.
Resilience and Maintainability Implications
observed — The new integrity exception is intentionally distinct from retryable contention. Audit persistence remains independent of business transactions, and representative callers either propagate audit failures or explicitly treat them as best effort. Those caller policies predate this change, but they determine the operational outcome of the new rejection.
Hardening Proposals
proposed — Before rollout, identify headless legacy chains and define an evidence-preserving adoption or repair procedure, with explicit operator authorization and audit-failure visibility. Do not restore availability by silently creating a new genesis over existing history.

Hash-check the complete chain before applying the time window.
When a window starts after an older interior event, the current loop checks only events. An altered chain_hash or previous_hash on that excluded event can therefore leave the selected events, tail, and sequence metadata valid, so the API reports valid: true for edited history. The documented verification contract says valid history must hash-link end to end and edited events must return valid: false.

Main (#43) put seq in the signed payload and still accepts legacy
seq-less signatures. The contiguity check assumed seq was unsigned and
rejected legacy rows with any stored seq. Only require contiguity across
seq-signed runs, and update tamper tests: renumbering a signed row now
fails the payload hash, and legacy seq-0 fixtures are signed seq-less.
Blocking: the legacy exemption in _first_sequence_break lets deletions at the legacy-to-seq-signed boundary verify as valid when created_after is used.
After a seq-less (legacy) row, expected_seq becomes None, so the first seq-signed row may carry any seq. Full-chain verification is still safe, because the previous_hash walk starts from genesis over every row. But windowed verification only hash-walks the window. It seeds previous_hash from the last surviving row below the window, then relies on the sequence check for out-of-window integrity, and that check is exactly what the exemption switches off at the boundary.
Repro (uncommitted scratch test, fixed values): 6 rows, seq 1-3 signed seq-less (legacy) and seq 4-6 seq-signed, all hash-linked, head at seq 6. Then verify_audit_chain(wallet_id=..., created_after=<created_at of seq 6>):
delete seq 4 (the first signed row): valid=True (undetected)
Full-chain verify (no window) catches every one of these with audit_previous_hash_mismatch.
On a pure seq-signed chain, the same "delete seq 4, window at 6" is caught as audit_sequence_gap. So it's the exemption itself that opens the hole.
Suggested fix (either works; the first is simplest and covers legacy interiors too):
Run the previous_hash / chain_hash link check over all_events from genesis regardless of the window, and keep the signature and payload checks window-scoped if cost matters. Any deletion anywhere then breaks a link.
Or, at minimum, anchor the first seq-signed row after a legacy run: require event.seq == previous_row.seq + 1, using the stored seq of the preceding legacy row, or legacy count + 1. Also require its previous_hash to equal the preceding row's chain_hash even when that row is outside the window.
Add a regression test for the windowed boundary deletion.
Otherwise: the renumber tamper tests expecting audit_payload_hash_mismatch are correct, because seq is signed since #43. The deleted-tail and missing-head checks are unchanged. test_audit_chain_tamper + test_audit_chain: 27 passed locally.
Re-review against my FAIL at 47f83d5. The fix (64b26f6) resolves it: _verify_single_chain now walks payload hash, previous_hash linkage and chain_hash over the full chain on one snapshot, checks signatures for rows in the window, and runs head/truncation plus _first_sequence_break (legacy-to-signed boundary must continue from the last legacy seq) on the whole chain regardless of window.
Delta: 64b26f6 touches only audit_chain.py, test_audit_chain.py, test_audit_chain_tamper.py. The merges af9e89d and 174c315: remerge-diff shows only a BREAK-REPORT.md conflict resolution (docs); no code hand edits.
Tests: test_audit_chain.py + test_audit_chain_tamper.py: 31 passed, including the parametrized deletions of row 4, row 3, rows 3+4 and row 2 under a window on row 6 (all invalid, audit_previous_hash_mismatch; the relinked row-3 deletion is caught by the boundary sequence check).
Extra adversarial cases I ran locally (scratch test, not committed), all with a window on row 6 over 3 legacy + 3 seq-signed rows:
delete row 5: invalid, audit_previous_hash_mismatch at row 6
tamper legacy row 2 metadata outside the window: invalid, audit_payload_hash_mismatch; with payload_hash and chain_hash recomputed: invalid, audit_previous_hash_mismatch at row 3
empty window (created_after a day past the last row): intact chain valid with checked_events=0; after deleting row 4: invalid; after deleting the tail rows 5-6: invalid, audit_chain_truncated
5 passed.
Note (non-blocking, by design): rows outside the window are not signature-checked, so a DB writer who also rewrites the head can re-hash rows after the window's end. A full-chain verify (no window) still catches this.
Delta review since my PASS at 174c315 (range-diff vs main: base commits unchanged; one new commit 099429b, no merges so no remerge-diff).
audit_chain.py: replaces the observation counter with a re-check of AuditChainHeadModel; only retries (_HeadConflict) when a head now exists (first-writer race), otherwise raises AuditChainHeadMissingError immediately (non-retryable).
New test deletes the head and asserts a single session attempt and a non-Contended error; uses runtime uuid4 wallet id.
Local: ruff clean, tests/test_audit_chain*.py 41 passed. CI green on head.
No linked issue; change matches PR description (sequence holes, deleted tail/head, blank wallet id now detected) and tests cover each case. Adds BREAK-REPORT.md at repo root (doc only).
Migration 017 set pre-seq rows to seq 0 and migration 019 created empty head
storage, so every append for such a wallet raised AuditChainHeadMissingError.
When no head exists and every stored row is a seq-0 legacy row, link the new
event to the terminal legacy chain hash at seq 1 and write the head. A
missing head over any seq-signed row still fails closed.

Head confirmed at ace9e19. Files: services/audit_chain.py, tests/test_audit_chain.py, new tests/test_audit_chain_tamper.py, BREAK-REPORT.md.
Wallets without sequence numbers continue at seq 1. In append_chained_audit_event, when there's no head row, the code reads the chain's terminal event. If every stored row is legacy seq 0, it links previous_hash to the terminal chain_hash and signs seq = 0 + 1 = 1. _first_sequence_break accepts a seq-signed run that continues after a legacy run. A test asserts the next append lands at seq 1 with head.last_seq == 1, and test_legacy_seq_zero_chain_without_a_head_still_verifies covers verify.
A missing head fails closed. If seq-signed rows exist but there's no head, verify returns audit_chain_head_missing, and append raises AuditChainHeadMissingError (non-retryable, tested) without inserting a new genesis. A head row that appears between the reads raises _HeadConflict and retries, so two first writers cannot fork the chain.
Double-spend and replay: sequences must now be exactly 1..last_seq across seq-signed rows. Renumbered gaps (1,2,4) and duplicates (1,2,2) return audit_sequence_gap. Verify now loads the head and the full chain in one snapshot even when time filters are set, then hash-walks every event, so a time window can no longer hide a deleted tail or a tampered link outside the window. NULL and "" wallet ids are folded into a single chain, matching the head key, which removes the split-chain false truncation.
Non-blocking, pre-existing: if an attacker deletes the head and every seq-signed row, the remaining all-seq-0 chain is indistinguishable from a legacy one. That residual risk is inherent to the seq-0 compatibility path.
Tests: 11 in test_audit_chain_tamper.py (middle deletion, windowed legacy and modern deletion, gap, duplicate, deleted tail across 3 windows, empty window, missing head on verify and on append, non-retryable error, blank wallet, legacy seq 0). The test_audit_chain.py expectation was updated to audit_chain_hash_mismatch because of the full walk.

What was wrong
The audit chain could be reported as valid after the tail event was deleted, as long as the verify call included a time window. Sequence numbers could be skipped or repeated and the chain still verified, because those numbers are not part of the signature and only the last number was compared to the head. Deleting the head row had the same result, and the next recorded event then started a second chain. A blank wallet id was stored on the same head as wallet-less events but checked as a different chain, so a real pair of events looked truncated.
Why it matters
Verify is the check operators use to trust the audit history. A valid result in these cases means a missing or reordered record can pass.
