What was wrong

The visitor report called a baseline sufficient whenever that column showed zero duplicate effects. A tool that never ran, and a baseline whose own verdict was FAIL, also show zero duplicates, so the report could say the existing integration was fine and hide a real duplicate from the other column. Five smaller checks had the same kind of hole: a boolean true counted as an execution, the word "false" in a claims file counted as agreement, a bundle index could point at a file outside the bundle, a non-ASCII lab control header crashed the route instead of being denied, and the effect snapshot was built from two separate database reads.

Why it matters

Those checks are what a reader uses to decide whether they need the gateway, and whether a downloaded bundle is the bundle they think it is. A pass built from a tool that did nothing, or from bytes that are not in the bundle, is the wrong answer.

How it was fixed

  • The report calls a baseline sufficient only when that column ran, its verdict is not FAIL, it executed the one intended action, and it produced no duplicate.
  • If every baseline that ran falls short of that, the conclusion is inconclusive. A real duplicate on another baseline is still counted as prevented.
  • The effect ledger snapshot takes one read and uses it for both the rows and the counts.
  • The bundle integrity check refuses absolute paths, parent paths, and symlinks, and it does not hash a file outside the bundle.
  • The receipt verifier accepts only a non-negative integer as an execution count. Boolean true and negative numbers are not counts.
  • A claims match flag counts only when it is the boolean true. The string "false" does not grant support.
  • The lab control-header check compares UTF-8 bytes and returns a denial if that compare cannot be done.

Tests

Added tests that lock each hole: test_a_baseline_that_executed_nothing_is_not_called_sufficient, test_a_failing_baseline_is_not_reported_as_sufficient, test_a_short_existing_column_does_not_hide_a_native_baseline_that_ran, test_only_a_non_negative_integer_counts_as_executions, test_a_non_boolean_match_flag_does_not_grant_support, test_snapshot_does_not_mix_two_reads, test_integrity_check_accepts_a_regular_file_inside_the_bundle, test_integrity_check_does_not_hash_a_path_that_leaves_the_bundle, and test_non_ascii_control_header_is_denied.

Before the code change, a direct Python script against the unfixed modules showed: a never-ran baseline concluded existing_integration_sufficient; a naive column with 2 executions plus a native FAIL with 0 executions concluded native_controls_sufficient; verifier executions: True was ESTABLISHED ("observed True execution(s)"); False and -1 were described as execution counts; claims matches_documented_expectation: "false" set support to true; snapshot() raised KeyError: 'refund:other' after two reads; integrity of ../outside.txt returned ok with checked 1; a latin-1 control header caf\xe9-not-the-token raised TypeError from hmac.compare_digest. A queued pytest of those tests never started: it slept in the shared slot queue for about 19 minutes and was stopped before it imported code, so the script is the before-fix evidence.

After the fix, through the slot queue:

cd gateway && ~/metacode/fleet/heavy /bin/zsh -c '/Users/sellers/metacode/fleet/venvs/gw/bin/python -m pytest -q tests/test_failure_lab_report.py tests/test_failure_lab_verifier.py tests/test_failure_lab_claims.py tests/test_failure_lab_evidence.py tests/test_failure_lab_harness.py -k "not lost_response" --tb=short; ... ruff check <the touched files>'

Result: 90 passed, 1 deselected in 6.72s, PYTEST_EXIT:0, ruff All checks passed!, RUFF_EXIT:0. The deselected test is test_lost_response_workload_across_configurations, which drives the real app and was not part of this change. Local ruff format --check on the touched files also reported they were already formatted.

Notes for the reviewer

A baseline that really executed the one intended action, and whose verdict is not FAIL, still gets the old "sufficient" conclusion. No migration. The diagnostic server is still loopback-only and unauthenticated, which is its design.

runner.py still says in its module comment that an ERROR column counts as having run. _view does not treat ERROR as ran. Left as is. backdate_attempts rewrites every attempt for the wallet, including rows that were never dispatched. Scenario T11 depends on that, so it was not changed. The control-header bug is reachable from a raw HTTP client sending latin-1 bytes. httpx rejects that header before the app runs, which is why the new test calls the ASGI app directly.

integration_check/judge.py, stateful.py, cli.py, telemetry.py, and most scenario bodies were not read line by line. Nothing in those files was changed.

Draft PR from the Muse Code QA fleet (instance qa5-am-gateway-failure_lab), reviewed by Grok Bot. Not merged or deployed; needs C.Lee's yes.

Note

Medium Risk
Changes how failure-lab reports and claims gate conclusions (more inconclusive, stricter booleans) and hardens bundle verification; behavior shifts for edge-case manifests and weak baselines but does not touch production gateway payment paths.

Overview
Tightens failure lab trust boundaries so visitors and CI cannot get a false “you’re fine” or “bundle verified” from sloppy inputs.

Visitor conclusions (report.py) no longer treat zero duplicate effects as proof a baseline handled the test. A baseline must PASS, run at least one intended downstream execution, and show no duplicates before existing/native sufficient applies; weak baselines (never ran, FAIL, OBSERVED-only) yield inconclusive instead, while real duplicate prevention on another column is unchanged.

Claims loading (claims.py) accepts matches_documented_expectation only when it is JSON boolean true, not strings like "false" or other truthy values.

Evidence integrity (evidence.py) adds path validation so manifest entries cannot escape the bundle via .., absolute paths, or symlinks; scans directories without following links; and validates manifest shape and read failures explicitly.

Effect ledger (effect_ledger.py) builds snapshot() from a single effects() read so counts and rows stay consistent.

Lab control API (refund_tool.py) compares the control header as UTF-8 bytes and returns denial on compare/encoding errors instead of 500 on non-ASCII headers.

Extensive pytest coverage locks each regression.

Review in Change Stack →

Walkthrough

The changes tighten manifest and file validation in evidence bundles, including checks for unsafe paths, symlinks, malformed entries, and filesystem errors. Failure-lab reports now require credible baselines with sufficient downstream executions before they establish sufficiency. Claim parsing accepts only a literal boolean match flag. Ledger snapshots use one effect read, and control-token checks reject invalid headers.

Priority: Normal

Estimated code review effort: 4 (Complex) | ~45 minutes

Merge Risk: Moderate · up to 2ea9f

Strengthen the bundle-integrity test so it detects invalid byte counts. Previously identified SDK, secret-scanning, documentation, and test-isolation concerns also remain unresolved; address or explicitly accept those risks before merging.

Security architecture risk: Minimal · up to 2ea9f

The change tightens evidence acceptance and protected control requests without adding privileges or service connections. No security issue was found to be introduced or materially worsened.

Retained concerns
No architecture-level concerns identified.

Security review details

Security Blast Radius

  • inferred — The relevant attacker-controlled inputs are bundle manifests and filesystem entries supplied for local verification, and requests reaching existing lab controls. The inspected changes tighten these inputs without granting additional filesystem privileges or control-token authority.

Trust Boundaries and Controls

  • observed — For stable filesystem contents, absolute paths, parent traversal, direct symlinks, and resolved paths outside the bundle are rejected before hashing. Symlinked roots and manifests are rejected. Tests exercise escaping paths and linked manifests as counterevidence to an introduced containment bypass.

Resilience and Maintainability Implications

  • observed — File-read and directory-scan failures contribute problems to a non-OK integrity result instead of silently establishing completeness. Repeated verification has no retained mutable verifier state that would bypass validation.

Hardening Proposals

  • proposed — If hostile concurrent writers are in scope, bind validation and reading to a stable filesystem snapshot or descriptor-based no-follow operations. Path replacement between checking and reading remains possible in both base and head; this is residual hardening, not an introduced PR concern.

Add a vector for the envelope-kid downgrade case.

The reference verifier verify_bundle in sdk/python/src/b2a_sdk/receipt_verifier.py handles one specific case. The signed payload names a kid that the verifier does not hold. The envelope kid names a key that the verifier does hold, and that key verifies the signature. The verifier returns MISMATCH for this case, not UNKNOWN_KEY. Its comment says that skipping this check lets a forger "downgrade a detectable inconsistency" into "refresh your key set".

No synthetic case tests this branch. synthetic.envelope_kid_mismatch covers only the reverse direction: the payload kid is held and the envelope kid is unknown. An independent verifier that skips the probe would still pass the whole suite.

Add one case to _synthetic_cases:

  • Sign a payload whose kid is "sk_unpublished" with the test key.
  • Set the envelope kid to KID.
  • Expect "mismatch".

Then add the case to REQUIRED_EDGES and REQUIRED_CASES.

The test name claims kid-downgrade coverage, but the test does not check it.

test_payload_hash_lie_and_kid_downgrade_are_rejected checks only synthetic.payload_hash_mismatch. It does not test a kid downgrade. A reader can believe that the verifier's envelope-kid probe has coverage when it does not.

Add the kid-downgrade assertion after the vector proposed in generate.py exists. Until then, rename the test.

292-292: 📐 Maintainability & Code Quality | 🔵 Trivial | ⚡ Quick win

The "payload without receipt_id" case does not reach the receipt_id check.

The bundle uses signature: "eA==", which decodes to 1 byte. verifyBundle rejects it at the 64-byte signature length check, before it reads receipt_id. The case passes for the wrong reason. The test asserts only that reason is truthy, so the signed payload has no receipt_id branch has no coverage.

Use a validly signed payload that omits receipt_id.

Use targetRules, not rules, to scope this allowlist to generic-api-key.

Gitleaks 8.30.1 documents targetRules for rule-specific allowlists. Without that field, the allowlist is global and can suppress other rules in these test paths.

Update the Gitleaks allowlist configuration to use targetRules instead of rules
for the generic-api-key rule, keeping the allowlist scoped to that rule rather
than applying globally.

Add the new reason codes to the consolidated reference.

Section 10 is the consolidated reason-code table. This PR adds policy_cost_invalid to it. The PR also adds daily_spend_unknown and risk_tier_not_allowed to the Layer C check table in section 5 and to denial-details.md. Section 10 does not list these two codes. A reader who uses section 10 as the full list will miss two denial reasons.

Restore every environment variable that the sqlite_store fixture sets.

The fixture now restores DATABASE_URL through _cleared_database_url. It still writes SQLITE_URL, STATE_BACKEND="sqlite" and REDIS_URL="" directly into os.environ and never restores them. Later tests in the same worker then construct DurableStateStore or Settings with an explicit SQLite backend and a temporary file path that has been deleted. That is the same kind of cross-test leak the DATARBASE_URL fix addresses. Use monkeypatch.setenv so pytest restores all four values.

🩺 Stability & Availability | 🟠 Major | 🏗️ Heavy lift

Do not run every call in a fresh event loop through asyncio.run.

Each method calls asyncio.run(...). Each call creates and then closes its own event loop. All calls share one httpx.AsyncClient (self._client). That client keeps a pooled connection that is bound to the loop that opened it.

On the second call, the client can reuse that pooled connection. The connection belongs to a closed loop. With a real transport, this commonly fails with RuntimeError: Event loop is closed. close() can fail the same way.

The MockTransport-based tests do not exercise real pooling, so they do not show this failure.

integrations/crewai-agent-middleware/src/crewai_b2a/tool.py already documents this hazard and keeps one persistent loop. Use the same approach here. Also, asyncio.run raises inside a running loop (for example, Jupyter), so detect that case and raise a clear error.

♻️ Proposed direction: one persistent loop per sync client
+    def _run(self, coro):
+        try:
+            asyncio.get_running_loop()
+        except RuntimeError:
+            pass
+        else:
+            coro.close()
+ raise RuntimeError("SyncAgentMiddlewareClient cannot run inside an event loop; use AgentMiddlewareClient")
+        if self._loop is None or self._loop.is_closed():
+            self._loop = asyncio.new_event_loop()
+        return self._loop.run_until_complete(coro)
+
     def discover_tools(self) -> list[ToolDefinition]:
-        return asyncio.run(self._client.discover_tools())
+        return self._run(self._client.discover_tools())

Initialize self._loop: asyncio.AbstractEventLoop | None = None in __init__. In close(), run self._client.close() on that loop, then close the loop. Replace the other asyncio.run(...) calls the same way.

Review comment at @sdk/python/src/b2a_sdk/sync_client.py at line 83:
Update the synchronous client methods that call asyncio.run, including
discover_tools, to reuse one persistent event loop per client for all async
operations. Initialize and manage that loop in the client, and have close run
the async client’s close operation on it before closing the loop. Detect calls
made while another event loop is running and raise a clear error directing
callers to the async client.

REVIEW: FAIL deepseek e82e8e3

dsh headless review of the full PR diff (merge-base..e82e8e3), run Oct 26 at 7:05 AM PT. This follows up my earlier hand review.

Findings

  1. Path containment bypass on Windows — gateway/failure_lab/evidence.py:1277-1291 and :1370
    _unsafe_bundle_path validates the path with PurePosixPath, but the file is later read with native root / relative. On Windows a manifest path such as C:/outside.txt is not caught by the leading //\ checks and is not absolute to PurePosixPath; candidate = root_real.joinpath(*pure.parts) becomes something inside the bundle, so relative_to(root_real) passes. Then root / "C:/outside.txt" discards the bundle root and _sha256_file at line 1379 hashes the file outside the bundle. A manifest listing C:/outside.txt with that file's digest can therefore make verify_bundle_integrity return ok=True for bytes that are not in the bundle. The existing test at gateway/tests/test_failure_lab_evidence.py:293-298 only uses str(outside), which contains backslashes on Windows and is rejected by the earlier check. Validate with native Path/PureWindowsPath, reject drive-qualified paths, and read the already-validated candidate rather than rebuilding root / relative.

  2. False explanation in the new INCONCLUSIVE conclusion — gateway/failure_lab/report.py:435-441
    This branch is reached for any non-credible baseline, not only one that executed zero times. For test_observed_single_effect_does_not_prove_existing_integration_sufficient (gateway/tests/test_failure_lab_report.py:264-276) both baselines executed exactly once, so “it executed the business action fewer times than this test requires” is false. For test_equal_duplicate_counts_do_not_claim_native_sufficiency (:242-261) both baselines executed twice and the test itself asserts duplicate_effects == 1, yet the conclusion says “A count of zero duplicate effects”. The tests only assert the conclusion kind/counts, so this inaccurate user-facing text ships. The message should distinguish “zero executions” from “verdict not PASS” and from “the baseline also duplicated”.

  3. The zero-execution regression test does not cover the zero-execution guard — gateway/tests/test_failure_lab_report.py:143-167
    The “executed nothing” test uses DIRECT_NAIVE=OBSERVED, 0 and DIRECT_NATIVE=FAIL, 0. Both are already rejected by the verdict == PASS check at report.py:307, so the test still passes if the column.downstream_effects >= INTENDED_EXECUTIONS condition at report.py:308 is deleted. The guard the PR claims to add is therefore untested. Add a PASS baseline with executions=0 (or otherwise isolate that condition) and assert it is not called sufficient.

REVIEW: PASS amw-qa 2ea9fa5

Re-review after DeepSeek FAIL (pullrequestreview-5653251714). All three findings are addressed.

  1. Windows / UNC bundle paths: _unsafe_bundle_path (evidence.py) rejects leading / or \, any ^[A-Za-z]: prefix (covers C:/x, C:\x, drive-relative c:x), any backslash anywhere (mixed separators), NUL, .. parts, symlinks, and resolve-escape. Parametrized test test_integrity_check_never_hashes_unsafe_manifest_paths covers C:/x, C:\x, c:x, //server/share, \\server\share, .., absolute, NUL, non-str. Forward-slash UNC is caught by the leading-/ check.
  2. INCONCLUSIVE text: gloss now reads "the measurements are not enough to say a baseline handled this test". The no-difference fallback returns INCONCLUSIVE instead of NATIVE_CONTROLS_SUFFICIENT.
  3. report.py guard: _baseline_is_credible requires ran + PASS + downstream_effects >= INTENDED_EXECUTIONS. test_pass_verdict_without_downstream_execution_is_inconclusive (naive + native, PASS with executions=0) asserts INCONCLUSIVE.

Also hardened: manifest shape validation, symlinked manifest/root, a non-following scandir walk, and int/bool byte checks.

Tests: gateway pytest -q -p no:randomly: 6179 passed, 89 skipped, 0 failed.
CI: all 20 checks pass (CodeRabbit skipped because the PR is a draft).

Nit (non-blocking): add an explicit mixed-separator case (e.g. sub/..\\x or a/b\\c) to the parametrized list. The code already rejects it.

Trivial · Maintainability & Code Quality · Quick win

Replace the bytes: "invalid" case with a test that reaches the byte-count validation.

The case [{"path": "file", "bytes": "invalid"}] has no sha256 key. The verifier computes expected = "", so actual != expected and the file lands in changed. The report is non-OK for that reason alone. The test still passes if the bytes validation at evidence.py lines 1394-1399 is removed.

The isinstance(expected_size, bool) branch has no coverage. If it is removed, "bytes": true still matches a 1-byte file because True == 1. The isinstance(expected_size, int) check also has no coverage. Add a test with a correct sha256 and a bad bytes value. The test must assert that changed is empty and that the problem comes from the byte-count check.

Based on learnings: validation tests should cover invalid types, and a bool must not pass as an integer counter. As per path instructions, security-critical changes should include negative-path tests.
