Summary

When a Claude Code subscription limit stops an unattended session, bmad-loop charged a dev attempt and eventually deferred a story whose spec was fine. This PR makes the orchestrator recognise the two captured limit shapes on the live pane:

  • Credit exhaustion (the CLI is blocked until someone acts): the session pauses with a usage limit: reason and is never charged.
  • Session-window limit (Claude Code waits for the reset and continues by itself): the session timeout stands still while it waits, so the session finishes after the CLI's own auto-continue.

Detection stays deterministic: it matches regexes against the visible pane and reads transcript idleness. There is no LLM in the loop and no new completion path. Every new exit is a stalled + parked non-completion with accept_result=False.

What changed, per phase

  1. Usage-limit pattern class and blocked detection (2ac82ca)
    • New profile fields usage_limit_patterns (blocked) and usage_wait_patterns (waiting for a reset), disjoint by contract and empty by default. Only claude is seeded, with one captured line each.
    • The pane is captured once at stall-grace expiry and checked against the parked patterns and both usage lists. A usage match withholds the nudge and ends the session stalled + parked with SessionResult.usage_limited set. When the session timeout fires, one capture against both lists sets usage_limited on the timeout result. A dead window is never usage-limited.
    • _classify_env_fault: a live usage_limited result is stamped env_fault with no token veto and no tail scan. The tail scan also covers both usage lists as ordinary families, and those stay subject to the veto.
    • New usage_limit_pause_reason, reached from env_fault_pause_reason and parked_pause_reason, so every dev, review, workflow, fix and sweep pause site inherits it.
    • The corpus gates (self-line, bare-cause, repo-wide scan, unseeded-profiles-inert, anchor literals) are generalised to all three pane-pattern fields.
  2. Live quota wait (38fb19f)
    • New policy key [limits] quota_wait_max_min → SessionSpec.quota_wait_max_s.
    • A wait is admitted at a heartbeat when all of these hold: the transcript has been idle ≥ 90 s, no tool call is open, the pane matches usage_wait_patterns, no QuotaPrompt/PermissionPrompt latch is set, and the session is under the cap.
    • While waiting, both deadlines are re-based on every tick, with at least 120 s left. A stall expiry re-arms the grace with no nudge. A Stop without a result withholds the stop nudge. An IdlePrompt latch never ends a wait.
    • The wait ends when the transcript moves or a blocking latch arrives. The cap is measured on both the monotonic and the wall clock, and hitting it ends the session through the shared stall exit (parked + usage_limited).
    • The token-budget grace is deliberately not frozen, because a crossed budget is real spend.
    • Surfaces: heartbeat.json gains quota_waiting / quota_wait_s; session-end gains quota_wait_s; the journal gains quota-wait-started / -ended / -cap. The new fields appear only when present.
  3. Reset-time parse and surfaces (904418c)
    • New pure, pyright-strict module usagereset.py. parse_reset(line, now) returns the next occurrence of H[:MM]am|pm, optionally preceded by a weekday or <Mon> <D> at and optionally followed by an (<Area/City>) zone. It never raises.
    • When the reset cannot be resolved, the raw text is kept and a usage-reset-unparsed crumb records why (unparsed or zone-unavailable).
    • The reset feeds SessionResult.usage_limit_resets, the pause reason suffix ; resets <value>, heartbeat.json quota_resets_at, session-end / workflow-end usage_limit_resets, and the TUI header, which shows quota wait <age> · resets <Ddd HH:MM> instead of idle.
    • If a resolved reset lies beyond the remaining cap, the session skips the wait and pauses at once (quota-wait-skipped).
  4. Docs and follow-ups (this commit)
    • Documented in the adapter-authoring guide (new "Usage-limit signals" section and field rows), FEATURES.md, the escalations guide (new "Usage-limit pauses" section), the run-state reference and the TUI guide. The CHANGELOG entries are tightened.

Why seeding alone was not enough

  • The token veto. The post-mortem classifier is overruled whenever the session's tracked usage is nonzero (A late transient 5xx classifies a fully productive dev session as an environment fault, pausing the run instead of retrying #764). A session that worked and then hit the wall would still be charged even with the line seeded as an env_fault_patterns family. The fix exempts a usage line from the veto only when it was seen live on the pane at the verdict.
  • The repainting wait. During Claude Code's auto-continue wait, the pane keeps repainting, so the stall timer never fires. Stall expiry is the only place the pane was looked at live, so the wait was never observed, while session_timeout_min kept counting: in the field capture, 73 minutes were blocked and the session timed out 17 minutes after resuming. The quota wait freezes the timeout for that stretch.

False-positive defenses

A false positive pauses a healthy run, so:

  • Anchors reproduce the captured sentence prefix (You're out of usage credit…, You've hit your session limi…). They never use a bare session limit, resets or usage credits. The model name is left out of the credit anchor because it varies by operator.
  • The advisory family cannot match. The same pane prints You've used <N>% of your session limit · resets … many times from a session that is still working. "hit your" versus "used N% of your" separates the two, and the advisories sit in the corpus BAIT set. Anchor-reaching bait is pinned too.
  • The veto exemption needs a live sighting. A usage line found only in the log tail stays under the veto (the stale-match rule), so a session that recovered from a limit and later stalled for a real reason is still charged.
  • Defaults stay byte-identical for every other profile. Both fields default to empty and the quota wait needs usage_wait_patterns. MockAdapter, resolve and probe build specs with the wait off.
  • The repo-wide corpus scan keeps either captured sentence from appearing unsplit in any tracked file outside the two corpus test files.

Policy key

[limits] quota_wait_max_min, integer ≥ 0, default 300 (one five-hour window). It sets the cumulative minutes one session may sit in a usage-limit wait with its timeout frozen. 0 turns the wait off, which restores the previous behaviour where the timeout keeps counting. The core.toml entry and the generated configuration reference are in sync.

Verification

  • Full suite: uv run pytest -q -n logical: 16918 passed, 116 skipped, 0 failed
  • uv run pyright: 0 errors
  • trunk check --all: no issues
  • uv run python scripts/gen_policy_reference.py --check: current
  • I ablated each proof-point test by deleting its gating code. Each test went red for the intended reason, and the source was then restored byte-identical:
    • test_stall_expiry_on_usage_limit_line_is_parked_and_usage_limited: usage branch of the stall gate
    • test_usage_limited_env_fault_is_not_overruled_by_tokens: veto-exemption branch
    • test_tail_only_usage_match_is_still_overruled: token veto
    • test_quota_wait_freezes_the_session_timeout: per-tick freeze
    • test_quota_wait_cap_ends_parked_with_usage_evidence: cap check
    • test_reset_beyond_cap_skips_the_wait: beyond-cap rule
  • The earlier phases ran their own ablations: 3 in S1, 10 in S2 and 7 in S3.

Follow-ups are filed in the internal ledger:

  • capture the remaining documented limit lines (weekly, per-model, the auto-continue footer, the rate-limit options menu);
  • consume Claude Code's StopFailure hook once a capture shows its error value;
  • timed auto-resume at the parsed reset;
  • opencode's quota patterns on the usage-limit class;
  • tzdata on Windows for zoned resets.

Supersedes #774 — thanks to @sirendhead and @nidelson for the captured lines.

Closes #610

Summary by CodeRabbit

  • New Features

    • Sessions can wait for a detected usage-limit reset, with a configurable per-session cap (default: 300 minutes; set to 0 to disable).
    • Usage-limit pauses include the detected limit and, when available, reset details and recovery guidance; they do not charge an attempt.
    • The run header displays quota-wait status and reset time when available.
    • Quota-wait duration and usage-limit details are included in run reporting.
  • Bug Fixes

    • Eligible quota waits pause session timeouts and stall nudges; sessions pause when the wait cap is exceeded or the reset falls beyond it.

…d of charging it (#610)

A Claude Code session stopped behind a subscription limit idled out its
clock and was charged a dev attempt, eventually deferring a story whose
spec was fine. Seeding the captured lines as env-fault patterns alone
would not have helped: #764's token veto overrules any post-mortem match
on a session that billed tokens before it hit the wall, which is every
such session.

Add the usage-limit pattern class: profile fields usage_limit_patterns
(blocked, no auto-continue: credit exhaustion) and usage_wait_patterns
(blocked until a reset the CLI continues from), disjoint by contract,
seeded in claude.toml from the lines captured in #610's runs. The wait
loop matches them against the VISIBLE pane at stall-grace expiry (one
capture shared with the parked gate; the nudge is withheld and the
session ends stalled + parked) and once when the session timeout fires.
A dead window is never usage-limited.

A live sighting sets SessionResult.usage_limited, and only that is
exempt from the token veto: the line is on screen at the verdict, so
tokens spent before the limit cannot disprove it. The same patterns
also scan the log tail as ordinary env-fault families, where the veto
still applies, so a session that recovered from a limit and later
stalled for a real reason is still charged. Both pause reasons name a
usage limit when the field is set; session-end and workflow-end carry
it present-only. The corpus gates now cover all three pane fields.

… usage-limit reset (DW-396)

Claude Code behind a session-window limit waits for the reset and continues
by itself, repainting its pane, so the stall timer never fires while
session_timeout_min keeps counting: the session timed out after the CLI
resumed and the story was deferred.

New policy key limits.quota_wait_max_min (default 300, 0 = off), handed to
every engine-driven session as SessionSpec.quota_wait_max_s. The wait opens
at a heartbeat when the transcript has been idle for 90 s, no tool call is
open, the visible pane matches usage_wait_patterns and no permission/quota
prompt is latched. While it is open both session deadlines are re-based on
every tick (the one sanctioned exception to #157's never-extend rule), stall
expiries re-arm without a nudge and a result-less Stop withholds the stop
nudge. Transcript movement or a permission/quota latch ends it; an
idle_prompt latch or the line scrolling away does not. At the cap, measured
on both clocks, the session ends stalled + parked with the usage evidence
through the shared stall exit. The token-budget grace is not frozen.

quota-wait-started / -ended / -cap go to the lifecycle crumbs and the run
journal; heartbeat.json gains quota_waiting / quota_wait_s, and
SessionResult.quota_wait_s rides session-end and workflow-end when positive.
…the heartbeat and the TUI

A new pure module, usagereset.py (pyright-strict), parses the `resets …`
clause of a matched usage-limit pane line: `[<Weekday> | <Mon> <D> at
]H[:MM](am|pm)[ (<Area/City>)]`, resolved to the next occurrence after an
aware local now, in the named zone when one is given. It never raises: an
unknown zone (no tzdata) or shape keeps the raw text and is crumbed
`usage-reset-unparsed`.

The reset rides SessionResult.usage_limit_resets (appended last) from every
usage-limited exit (stall gate, timeout look, quota-wait cap), the pause
reason (`; resets <value>`), session-end / workflow-end (present-only),
heartbeat.json `quota_resets_at` while waiting, and quota-wait-started
`resets_at`. A resolved reset beyond the remaining cap skips the wait and ends
stalled + parked at once (crumb quota-wait-skipped, reason=reset-beyond-cap),
never a completion. The TUI header renders an open quota wait as
`quota wait <age> · resets <time>` instead of the idle notice.

Full details: Title check

Explanation

The title names the main change: Claude Code usage-limit handling. It says the run pauses on those limits and waits out the reset without charging attempts. The raw summary and objectives describe usage-limit detection, pausing, and quota-wait handling as the core work, so the title matches the changeset. The title uses a conventional commit prefix and references issue #610. The title is 107 characters, which is longer than ideal, but it stays specific and readable.

Full details: Linked Issues check

Explanation

Direct issue #610 requires seeding a Claude Code usage-limit pattern from a captured CLI line, citing the source run, with corpus safeguards against healthy output. The head seeds the claude profile with usage_limit_patterns (credit exhaustion) and usage_wait_patterns (session-window limit). Each seed is a captured sentence prefix with a source reference. tests/test_env_fault_patterns.py holds the captured lines with provenance, adds bait that reaches the anchors, and extends the corpus gates (self-line, repo-wide scan, unseeded-profile inertness) to all three pane-pattern fields. Deviation: #610 step 2 names env_fault_patterns. The PR uses two new disjoint fields instead, and states the reason: the token veto would still charge a session that worked and then hit the limit, so a live-pane exemption needs its own field. The requirement is met in substance. The incremental commit 1371db9 changes timeout verdicts so that a working session's timeout is kept out of the usage-limit verdict. This does not remove the seeded patterns or the corpus gates. Limit: this review did not reread every named gate test body, and it did not inspect the 1371db9 diff line by line. The prior PASS is preserved on the basis of the PR's reported green run and the prior review.

Full details: Out of Scope Changes check

Explanation

#610 asks only for seeding a captured usage-limit pattern with corpus safeguards. The live-pane usage-limit pause, the token-veto exemption, the usage-limit pause reason, and the corresponding corpus tests serve that goal and are in scope. The following changes are not required by #610, and the issue text does not mention session timeouts, reset parsing, or TUI display: (1) the quota-wait feature: the [limits] quota_wait_max_min policy key and its default of 300, SessionSpec.quota_wait_max_s, the wait logic in src/bmad_loop/adapters/generic.py that freezes session timeouts and withholds stall and Stop nudges, and the quota-wait-started, quota-wait-ended and quota-wait-cap journal events; (2) the reset-time subsystem: the new src/bmad_loop/usagereset.py module, the usage_limit_resets fields, the quota_resets_at and quota_waiting heartbeat fields, and the quota-wait and reset fields in session-end and workflow-end entries; (3) the TUI changes in src/bmad_loop/tui/widgets.py and src/bmad_loop/tui/data.py; (4) the pyproject.toml strict-typing entry for usagereset.py and the documentation for these features in FEATURES.md, the run-state, TUI, configuration and escalations references, and the adapter-authoring guide. The PR description still closes #610 and presents these features as part of the same change, so the scope has not changed since the previous review.

Resolution

Move the quota-wait feature (policy key, wait loop, timeout freezing, quota-wait journal events), the usagereset.py parser and its surfaces (heartbeat, journal, session-end and workflow-end fields, TUI header), and their documentation and tests into a separate issue and pull request that links its own requirements. Keep this pull request to the usage_limit_patterns and usage_wait_patterns fields, the live-pane usage-limit pause and token-veto exemption, the usage_limit_pause_reason path, and the corresponding corpus tests and documentation. Remove the reset-time suffix from the pause reason if it stays with the usage-limit pause. If the maintainers decide that quota waiting belongs under #610, record that decision as an explicit scope change on #610 first, and update the PR description to match.

P2 Badge Check current activity before labeling a timeout usage-limited

When a session recovers from a usage limit but its old limit line remains visible in Claude's inline renderer, this timeout path still sets usage_limited, even if the transcript is actively growing or a foreground tool is running. The new test_quota_wait_needs_an_idle_transcript explicitly models this recovered-and-working scenario, but only checks that the timeout fires—not its classification. Because _classify_env_fault unconditionally exempts usage_limited results from the token veto, a genuine working-session timeout now pauses the run without charging an attempt and incorrectly tells the operator to resolve a usage limit. Apply current-activity checks here, rather than treating visibility alone as proof that the CLI is still blocked.

Fixed in 1371db9. The timeout path now applies the quota wait's activity tests before it takes the pane's word. If the transcript was written within QUOTA_WAIT_IDLE_S (as of the last heartbeat sample), or a tool call is still unanswered, the session counts as working and its timeout is an ordinary charged one. A session with unknown activity (no transcript named yet) keeps the pane label. test_quota_wait_needs_an_idle_transcript and test_quota_wait_not_admitted_with_an_open_tool_call now also assert usage_limited is None, and each new guard was ablated red. The adapter guide and FEATURES are updated to match.

…t verdict

A usage-limit line left on screen by a session that recovered and kept
working labelled its later timeout usage-limited, which the env-fault
classifier exempts from the token veto: the run paused without charging and
asked the operator to resolve a limit long gone. The timeout path now applies
the quota wait's activity tests (transcript written within QUOTA_WAIT_IDLE_S,
or an unanswered tool call) before taking the pane's word.

P2 Badge Preserve local timezone rules when resolving future resets

When a reset without an explicit zone crosses a daylight-saving transition, astimezone() supplies a fixed-offset datetime.timezone, so parse_reset() applies today's offset to the future reset. Reproduced with TZ=Europe/Warsaw: at 2026-03-29 00:30, resets 6am resolves to 06:00+01:00 instead of 06:00+02:00. This makes the reset appear 5.5 hours away rather than 4.5, causing the reset-beyond-cap branch to pause immediately under the default five-hour cap instead of waiting. Resolve future local wall times using the local timezone's transition rules rather than retaining only its current offset.

Fixed in b41d385. _local_now() now returns a naive local datetime. When the clause names no zone, parse_reset resolves each candidate wall time with astimezone(), which applies the host zone's transition rules instead of today's fixed offset. The reset-beyond-cap comparison now does reset.at - local_now.astimezone(). New tests, using a POSIX TZ string so no tzdata is needed: 6am on the spring-forward night resolves to 06:00+02 (4.5 h away), a weekly Mon 12:00am reset across fall-back keeps its wall time, and the adapter's _local_now stays naive. Both ablations (a fixed-offset resolution, an aware _local_now) turn those tests red.

…zone's DST rules

_local_now() handed parse_reset an astimezone() datetime, whose fixed
offset was stretched over any DST change before the reset: a weekly
'Mon 12:00am' reset, or a 6am reset on the spring-forward night, came out
an hour off and could trip the reset-beyond-cap rule. _local_now() is now
naive and parse_reset resolves each naive candidate wall time with
astimezone(), which applies the host zone's transition rules.

Require fresh limit evidence before reopening a quota wait.

If the transcript resumes after a reset but the old limit line remains in the captured pane, a later 90-second idle period can reopen a wait from that old line. If its reset time has passed, _usage_reset can resolve the next occurrence beyond the remaining cap, and Line 1738 parks a session that has not hit a new limit. Invalidate the prior pane match when transcript activity resumes, and admit another wait only from new limit evidence.

A wait that ended because the transcript moved left its limit line on
screen in Claude's inline renderer. At the session's next idle stretch
that stale line could reopen a wait, or, once its reset had passed and
resolved to the next day, skip straight to a parked stall. It also
labelled a later stall or timeout usage-limited. The matched line of a
wait the CLI continued from is now spent (_QuotaWait.spent), and
_pane_evidence skips it at admission, stall-grace expiry and the timeout
look. A new limit prints a new line, which still opens a fresh wait.

Re CodeRabbit outside-diff Major on 1371db9 ("Require fresh limit evidence before reopening a quota wait", generic.py:1711-1714): fixed in c70af79. When a wait ends because the transcript moved, its matched pane line is recorded as spent (_QuotaWait.spent). _pane_evidence skips spent lines (compared stripped) at all three live usage gates: wait admission, stall-grace expiry and the timeout look. A stale line on screen therefore cannot reopen a wait, cannot park the session through reset-beyond-cap once its reset has rolled to the next day, and cannot label a later stall or timeout usage-limited. A new limit prints a new line with its own reset, so it still opens a fresh wait. New tests: test_spent_wait_line_does_not_reopen_a_wait, test_spent_wait_line_does_not_park_a_stall and test_fresh_wait_line_after_a_spent_one_opens_a_new_wait. I ablated each skip, plus an over-broad skip-everything variant, and each turned a test red. The adapter guide documents the rule.

P2 Badge Retire recovered limit lines even before a quota wait opens

If a limit resets before the transcript has been idle for 90 seconds, the CLI can resume without ever opening a quota wait. Its old limit line is therefore never added to quota.spent. When that session later becomes idle for 90 seconds, admission matches the leftover line, resolves its already-passed reset as tomorrow, and immediately returns stalled + parked through reset-beyond-cap under the default five-hour cap. This incorrectly pauses a recovered session instead of allowing its normal timeout/stall handling. Track recovery from observed limit lines even when no wait was admitted, rather than retiring lines only when an open wait ends.

Declining this one as an edge case. To happen, the limit has to reset inside the admission window: the transcript must stay idle for QUOTA_WAIT_IDLE_S (90 s), and the heartbeat samples every 30 s, so about 90–120 s after the line prints. That is under 1% of five-hour session-limit hits. The line must also survive on the visible pane through the work the CLI resumes, and the session must then go 90 s with an idle transcript and no open tool call. Closing the gap needs a pane capture on every active heartbeat, so a line's first sighting can be dated and its reset resolved against that time instead of now. That is a standing cost on every Claude session for a case this narrow. If it does happen, the run stops on a visible usage limit: pause with the evidence and reset time, which the operator resolves. Nothing is lost. The other half of this round (stall-time classification, 10c9154) does close the more likely variant: a recovered line on a session that is still working.

P2 Badge Apply the activity guards to stall-time usage classification

When a recovered session leaves its old limit line visible and then runs a silent foreground tool beyond the stall grace, quota admission correctly refuses to wait because a tool call is open, but this stall branch still unconditionally sets usage_limited. Unlike the corrected timeout branch, it never checks transcript activity or _quota_wait_tool_open. Consequently, _classify_env_fault bypasses the token veto and pauses the run as usage-limited without charging an attempt, although the limit has already cleared. Apply the same current-activity guards here before treating the visible line as live usage-limit evidence.

Fixed in 10c9154. The stall gate now runs the same activity tests as the timeout path. A transcript written within QUOTA_WAIT_IDLE_S (as of the last heartbeat sample) or an unanswered tool call counts as a working session, and so does a tool scan that faults. Both gates share them through one closure, still_working. On a working session a visible usage line no longer sets usage_limited at stall-grace expiry, so the stall stays ordinary (nudge or charged stall, as before #610) rather than pausing the run uncharged. If no transcript is named yet, activity is unknown and the gate still takes the pane's word. New test: test_working_session_stall_is_not_usage_limited (cases: open Bash call, moving transcript). Removing still_working() from the stall gate turns both cases red. The adapter guide and FEATURES.md now say both gates apply the rule.

…verdict

A usage-limit line left on screen by a session that recovered with no quota
wait ever admitted labelled a later stall usage-limited when the pane went
quiet past the grace on a silent foreground tool or a still-moving transcript.
The env-fault classifier exempts that label from the token veto, so the run
paused uncharged for a limit long gone. The stall gate now applies the same
activity tests the timeout path does (transcript written within
QUOTA_WAIT_IDLE_S, or an unanswered tool call), shared as `still_working`.
