Form URL copy breaks connection

High Severity · Logic Bug

The setup form now titles DATABRICKS_HOST as Workspace URL, claims a workspace or Genie MCP URL is accepted and only the hostname is kept, and shows an https:// example. That value is still substituted verbatim into https://${DATABRICKS_HOST}/... with no hostname extraction, so following the form produces a malformed MCP URL and the plugin cannot connect.

Added placeholder examples for `DATABRICKS_HOST`, `CLIENT_ID`, and `CLIENT_SECRET`. Variable keys, types, and required fields are unchanged; `CLIENT_SECRET` stays optional for public clients.
