Focused validation branch for upstream issue openai#4775 / PR openai#4906. The branch is based on upstream main commit 89c02c8 and contains the pending-input recovery implementation plus focused regressions for provider_data round-trip/lost-ack recovery. This fork PR is only for running CI and auditing the patch; it is not proposed as a replacement for upstream PR openai#4906.
