The PR description says this PR adds docs/conventions/pr-pipeline/refine-canary.md, but the file is added at docs/refine-canary.md. If the pr-refine / fix-docs run, or the record of it, expects the documented path, it won't find the file here. Either move the file to the described path or correct the "Fix" section of the description.

The refine stage tidies a pull request before review. It runs once per pull request.
