Thanks for writing this up — the four-tier structure reads well and the error strings match the implementation exactly, including the case where the — <provider error> clause is omitted. I checked the 400, the "no changes are saved" behaviour, and the managed-identity exclusion against the code and they all hold.
I have one point on the Enterprise note that I think may not be quite accurate, plus a few optional suggestions about scope. Happy to be told any of them are unnecessary.
